Would you like to work with a forward thinking and innovative team in Community Nursing? If so, Portsmouth Community Nursing Team are recruiting!

This is an exciting opportunity for a motivated nurse to join the Community Nursing team in Portsmouth as a Tissue Viability nurse. We are looking for a dynamic enthusiastic nurse who has experience in, or a genuine interest in tissue viability and leg care to join the team. The team participates in the provision of specialist nursing care to clients and provides specialist advice to clients, carers, and the multidisciplinary team.

Ideally applicants should have Tissue Viability experience within a community setting or equivalent relevant experience in an acute setting, with evidence of continued professional development. Support will be offered to develop current skills and gain expertise in other areas of wound care. Experience of delivering education would be beneficial as you will be required to provide education in the classroom and informal settings. Acting as a resource to fellow professionals, carers, and clients in a range of settings, you will need effective interpersonal skills and show leadership skills to support a flexible and positive approach to problem solving.

You will also need a driving licence with access to a vehicle for work use (mileage allowance paid).

Main duties of the job

Assess, plan, implement and evaluate specialist care for clients adopting best practice standards for clinical care

Develop individual education/support programmes, providing expert advice on management of Tissue Viability issues in order to achieve optimal clinical outcomes and improve patients quality of life

Act as a resource to fellow professionals/carers/clients in the community setting

Collaborate and communicate with members of the multidisciplinary team to ensure provision of an optimum level of service to clients and their carers

Act as a resource in the care of clients at risk of or with tissue damage and in the field in Tissue Viability

Contribute to initiatives such as the development of the service e.g., chronic wound management under the direction and guidance of the Clinical Lead

Maintain good communication with patients/clients/carers and families ensuring that their individual needs are met, and address any issues identified
