Job summary

Are you passionate about providing a patient-centered high quality service to people with Heart Failure? An exciting opportunity has arisen in our Community Heart Failure Nursing service for a part-time Senior Heart Failure Nurse.

The Community Heart Failure Service works as part of Adult Specialist Services Portsmouth to provide specialist heart failure management and treatment for patients and offers a specialist resource for professionals, families, carers, and staff from other agencies involved in their care. The role of the Heart Failure Nurse is to manage and support patients with a confirmed diagnosis of Left Ventricular Systolic Dysfunction (LVSD).

The Community Heart Failure Service is based at St Mary's Hospital in Portsmouth. The team provides heart failure management in the community and patients are supported in a range of settings, including their own homes, residential homes, and clinics.

This is a rewarding and challenging role caring for patients with long-term life-limiting conditions.

You will be an experienced nurse holding a professional registration with the nursing and midwifery council and with cardiology experience.

For this post, you will need to hold a full UK driving license and have access to the use of a car.

Main duties of the job

To meet the needs of patients and their families to achieve improved health outcomes and promote health and well-being while demonstrating care, compassion, competence, communication, courage, and commitment.

Be responsible for coordinating the day-to-day work of the heart failure team, influencing clinical decisions, and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice, underpinned by theory and expertise.

Supervise junior members of staff, students on placements

Prioritise and undertake initial assessments providing specialist advice and expertise in the care of those with a heart failure diagnosis.

Work as an Independent Non-Medical Prescriber to prescribe medication (once qualified to do so) according to patient need adhering to the guidelines from the National Prescribing Centre, NICE, and Solent NHS Trust Policy. Or alternatively, as part of the management of complex symptom control provide advice on the tailoring of pre-prescribed medications in response to changing clinical needs.

Working in partnership with patients, carers, and service users, you will advise the team with responsibility for providing the highest standards of patient care through ongoing assessment, planning, implementation, and evaluation of programmes of care. Deal empathetically with sensitive information, including discussing the patient's changing condition with the patient and/or care.
