Main duties of the job

The role involves delivering highquality, evidencebased nursing care to patients within their own homes, many of whom have complex and evolving needs. Our District Nursing teams work collaboratively with specialist nurses, GPs, Therapists, and Social Care colleagues to provide coordinated support for patients and their families, helping to prevent avoidable hospital admissions and promote independence.

We are seeking highly motivated, enthusiastic, and forwardthinking individuals who can embrace the challenges and opportunities within an everdeveloping primary care environment. Previous community experience is not essential -- full training, guidance, and ongoing support will be provided to help you grow confidently into your role as a Community Nurse.

The Trust offers an excellent preceptorship programme, complemented by our new local induction for community services and the allocation of a dedicated buddy mentor within the team.

To meet the needs of the service, you will need to be able to work flexibly between 07:00 and 23:00 on a rota basis across seven days, and you must be able to drive. We actively promote flexible working and will consider a range of working patterns to support a healthy work--life balance.
