An exciting opportunity has arisen at the West Midlands Adult Cystic Fibrosis Centre for a qualified social worker to join the large and varied This is a full-time role, working Monday to Friday. Support and training will be provided.

This is an exciting time for Cystic Fibrosis Services with the recent introduction of CFTR modulators, which have significantly improved the health of many patients, though not all. The role requires an understanding of the impact of living with a life limiting, chronic disease and the psycho-social issues that can result because of it.

Although some of the work is based on crisis intervention, there is also the need for both short-term problem focused work, and longer-term therapeutic work which makes it an interesting and varied role.

The successful candidates will be supported by the Lead CF Social Worker and the wider multi-disciplinary team to develop their knowledge and skills to work with patients experiencing a wide range of problems associated with their chronic health condition.

The successful candidate will need to be registered with Social Work England and hold a valid driving licence.

The social worker will provide a comprehensive service to individuals receiving care from the team, as well as to their carers and families when relevant. After assessing each person's needs, the social worker will offer support regarding the social, emotional, physical and financial impact of their condition or disability. This support will be proactive and cover a variety of issues, including access to resources, community care, employment or education, fertility treatments, childcare, transplantation, end-of-life care, and bereavement.

Additionally, the social worker will assist individuals with extra vulnerabilities, such as learning disabilities, mental health issues, substance misuse, or other challenges.

Qualifications

Essential

  • * Social Work qualification in Certificate of Qualification in Social Work (CQSW), Diploma in Social Work (DipSW) or equivalent relevant experience
  • * Evidence of current registration with Social Work England and evidence of ability to maintain practice requirements.

Desirable

  • * Post qualifying qualification e.g. ASYE, Practice Educator, Best Interests Assessor

Experience

Essential

  • * Post-qualifying experience in assessing vulnerable individuals using personalised care principles to identify their needs, strengths,challenges, and risk areas, and coproducing support plans to meet those needs.
  • * Managing complex cases and involvement in multi-agency work.Experience of working in a multidisciplinary team
  • * Direct experience of working with adults affected by chronic illness and/or physical disabilities
  • * Has understanding / experience of the principles of Safeguarding in relation to the role

  • * Experience of working with patients with a life limiting condition preferably in a clinical setting
