Band 3 Clinical Support Worker – Community Phlebotomy

Job summary
Total Hours 37.5hrs ( 18.75 x2 split) .
We are looking for a confident and compassionate person to join our team supporting haematology patients at home. You will be a skilled and confident phlebotomist, confident in communicating with patients and their families in their own home. You will ideally have experience of palliative care and caring for patients at the end of their life, although full training and support will be given. You will be able to complete and record patients vital signs and know when to act to raise concerns and seek help. You will be confident to use I.T equipment and systems. You will be able to organise your own workload with support, but also be comfortable feeding back to the nursing team and raising concerns . A full driving license and access to your own car is essential.
Develop individual competencies and knowledge which will support the provision and evaluation of seamless specialist nursing services, ensuring patients receive the highest standards of clinical care. You will be supported by a more experienced nurse or nurses to enable you to achieve competence in agreed areas of specialist practice through exposure, training and education.
Main duties of the job

Based within a site to include working in a range of practice settings across health and care providers. Additionally, you will be required to practice in premises/settings outside of NHS care providers as part of their role and travel to and from a range of practice settings across an agreed geographic, this may include secure units/residential homes and other regulated settings where patients may reside
MAIN DUTIES & TASKS:
- Attend patients home to collect blood samples: phlebotomy
- Carry out patient vital signs if needed
- Communicate with the wider team/patients/relatives/primary care and other community teams/carers
- Delivery of samples to laboratories
- Carry out risk assessments of lone working and home visits - with support
- Documentation of visits and any concerns
- Work as part of a nurse-led team to deliver high standards of care for patients at home
