Band 6 Falls Specialist Therapist OT/PT – (Inpatient and Community)

Job summary

An exciting opportunity has arisen for a Band 6 Therapist (Occupational or Physiotherapist) within UHB working in a hybrid role across the inpatient wards and Solihull Community Specialist Falls Team. The post holder will work within both inpatient and community teams helping to deliver specialist advice in reducing falls risk across a wide variety of specialities. The post holder will be a dynamic, caring and enthusiastic, highly motivated individual with experience of falls prevention and management in a clinical/community setting. The successful post holder will be prodominently based at Solihull Hospital (50%) and then working across Solihull Community (50%) supporting patients in their own homes. They will however need to demonstrate a flexible approach to working across all hospital sites as required. The post requires a high level of communication and engagement with other key stakeholder groups to successfully provide an effective and efficient falls service across the acute to commmunity pathway for Solihull patients.

Main duties of the job

This post is a split post;

1) 50% working within the Falls Inpatient UHB team (prodominently ward based on site at Solihull Hospital)

2) 50% working within the Solihull Community Specialist Falls Service (based on site at Solihull Hospital, traveling across Solihull to carry out home based assessments)

The post holder will be passionate about ensuring clinical teams are well supported by acting as an expert resource on falls prevention and management to staff, patients and their families/carers. This will include delivering training, undertaking patient reviews, management of clinical / liaison work and risk management, and being a key adviser for Clinical teams including a falls advocate for Therapies. The post holder will also provide specialist falls assessments and intervention plans for patients living in their own homes who are at risk of or who are experiencing falls. The post holder will demonstrate a 'can do' attitude, show commitment to driving quality improvements across a wide and varied organisation, and practice new and innovative ways of working to improve quality and patient safety. As an integrated pathway for falls we strive to ensure that our patients can expect high standards of falls risk identification, and in turn receive the right falls prevention interventions at the right time and in the right place, wherever they are in the organisation.

Qualifications

Essential

  • * Registered Physiotherapist or Occupational therapist
  • * BSC in Physiotherapy or Occupational Therapy or equivalent qualification /training or experience.
  • * Professional knowledge acquired at degree level supplemented by specialist training relevant to the specialism to postgraduate diploma level or equivalent

Desirable

  • * Post registration qualification in (appropriate area of speciality)

Experience

Essential

  • * Experience of working at Band 5 level in a relevant area of the NHS
  • * Experience in working in both inpatient and community settings
  • * Significant experience at Band 5 level with evidence of passion/ enthusiasm and knowledge for areas of specialist practice.
  • * Evidence of achievements and career development in current post and of ongoing professional development/ competence.
  • * Demonstrate specialist knowledge underpinned by theory and CPD relevant to speciality.
  • * Experience and enthusiasm for teaching / assessing and supervising others in a clinical setting.
  • * Awareness of current relevant NHS policy context relating to the speciality.
