Job summary

We are looking for an experienced and inspirational leader to manage the day-to-day operations of our Critical Care Follow-Up Service. This role includes responsibility for service development, clinical governance, staff development, and training, ensuring the highest standards of professional practice and patient care.

You will provide expert clinical leadership, supervise practice, audit, research, and teaching to continuously improve care quality and patient experience. The service delivers comprehensive follow-up care for patients recovering from critical illness, addressing physical, psychological, and social needs in line with NICE guidance and national best practice.

The successful candidate will combine advanced clinical skills with strong leadership and operational management, ensuring safe, effective care and optimal use of resources. Main duties of the job

Communication & Relationship

  • Actively engage patients, families, and carers through feedback mechanisms, support groups, and open forums to inform service development.
  • Communicate complex and sensitive information with empathy, tact, and confidentiality, adapting methods to meet individual needs.
  • Lead and participate in team meetings, ensuring robust communication processes internally and externally.
  • Maintain visibility within the service, promoting transparency and a caring environment that respects individual values.
  • Provide bereavement support and facilitate critical care visits as part of recovery.
  • Network across professional groups and represent the service at internal and external meetings.

Analytical & Judgemental

  • Manage an autonomous caseload, delivering personalised, evidence-based care.
  • Act as a patient advocate, applying critical analysis and decision-making to influence service improvements.
  • Monitor care quality and advise on clinical decisions using expert knowledge of critical care recovery.

Planning & Organising

  • Coordinate follow-up pathways, including nurse-led clinics, telephone consultations, and ward rounds.
  • Ensure safe transitions between critical care, ward, and community settings.
  • Lead workforce planning, staff rostering, and maintain appropriate skill mix.

Physical Skills

  • Demonstrate advanced clinical skills, that could include tracheostomy and laryngectomy care post-ICU discharge.
  • Use IT systems and multimedia tools for documentation, education, and communication.
