Exciting Career Opportunity for Physiotherapists!
Join our Solihull Community Services Intermediate Care team, working across multiple sites in Solihull and Birmingham. We provide therapy for patients needing further assessment and rehabilitation after a hospital discharge, to determine the appropriate community pathway/discharge destination for our patients.
Our main focus is supporting complex discharge planning and ensuring smooth transitions from hospital to community care.
This role offers the chance to deliver physiotherapy in collaboration with a skilled, supportive team committed to professional growth and development. We provide a 7-day therapy service, including weekend cover, and the post holder will join the respiratory on-call rota for UHB hospitals.
Ideal for an experienced Band 5 seeking progression or a Band 6 looking to expand expertise in managing complex discharges. Driving and access to a vehicle with business insurance is essential.

Main duties of the job
*To deliver a high standard of therapy provision within the Solihull Community and Intermediate Care Bedded Units across Pathway 1 and 2.
*To support our patients in their recovery journey and minimise the risk of hospital readmissions.
*To make the transition from acute care to the community as smooth as possible, ensuring timely discharges and efficient flow.
*To liaise with various agencies to ensure safe, effective and efficient discharges.
*To provide Physiotherapy services in the patient's own home or residence.
*To contribute to seven-day working programmes across the organisation.
*To be part of On-call respiratory Rota
*To be able to manage your caseload and work independently.
*To supervise students on placements.
*To appraise, supervise and support junior staff and therapy assistants.
*To initiate/participate in audits, research and quality improvement programmes.

Essential
- *Good standard of general education including GCSE English and Maths grade C or above or equivalent
- *A Levels or equivalent qualification
- *Degree/diploma in Physiotherapy
- *Registered with Health and Care Professionals Council
- *Evidence of CPD including recent post-graduate training relevant to clinical speciality
Desirable
- *Evidence of leadership training
- *Fieldwork Educator's certificate or attendance at Clinical Educator Course
Experience
Essential
- *Experience of working in an Intermediate Care rehabilitation setting.
- *Demonstrates clinical experience enabling the post holder to manage a caseload of patients with complex needs.
- *Post grad Experience in Medical, Older People, Trauma and Orthopaedics & Respiratory
- *Specialist knowledge across a range of practices, routine and non-routine, requiring expertise within the field of Physiotherapy relating to Community Services.
- *Previous and ongoing attendance of mandatory training
- *CPD portfolio
Desirable
- *Experience and understanding of team/service development
- *Supervision of Band 5 staff, students and assistants
- *Clinical experience within Community Therapy Services including admission prevention service
- *Previous and ongoing attendance at recent postgraduate courses relevant to the specialist clinical work
