Job summary
Ward 9 are seeking a band 4 flow coordinator to join the team,
Ward 9 is one of health care for older adults wards here at Good Hope, its a 33 bedded mixed sex ward.
are you able to demonstarte excellent communication skills work well within a muliti disciplinary team able to utilisation of the internal escalation process and ensuring the multi-disciplinary team adhere to the Department's internal professional standards.
The post holder will ensure the timely flow of patients through the area to help identify available capacity and place patients appropriately, working within agreed procedures and protocols.
Contracted hours - 15hours
Main duties of the job
With a focus on urgent and emergency access and inpatient stay the post holder will work closely with the clinical and operational management teams ensuring all patients referred to and cared for by the speciality are tracked and have treatment and management plans and that there are effective discharge or transfer plans in place. They will maintain accurate and comprehensive records of patient information which relates to admissions and discharges (planned and unplanned, including intra hospital transfers and urgent referrals and internal referrals), work closely with families to support patients to a sfae discharge destination
