B7 CNS Team Manager: Liver Disease Detection Project

Job summary

This is an exciting opportunity for an enthusiastic, organised and well-motivated individual to work as part of the project team, based at UHB, to identify patients in Birmingham area who are at risk of developing liver disease and liver cancer.

Birmingham's rate of mortality from alcoholic liver disease in persons less than 75 years is 14.8 per 100,000, which is higher than the national average.

Most liver disease is preventable, and much is influenced by alcohol consumption and obesity prevalence, which are both amenable to public health interventions.

This is a Birmingham Initiative to improve the detection of alcohol and obesity related liver disease to prevent early deaths. Working in partnership with the University Hospital Birmingham (UHB) liver team, Birmingham City Council (BCC) has funded a Fibro-scanning programme to help identify liver damage at an earlier stage when it is more likely to be reversed with lifestyle changes or treatment.

The post holder should have excellent communication skills to co-ordinate patient care amongst many professionals. A key responsibility will be to build good working relationships with primary care teams, to facilitate the liver health screening programme.

The post holder will manage the Band 6 CNSs to deliver on the programme; to check for liver damage in high-risk communities so measures could be taken to prevent its progression and to help with returning the liver to good health.

Main duties of the job

The post holder will use their clinical skills to assess qualifying patients for evidence of liver damage using several testing methods for diagnosis, such as taking blood tests and performing fibroscans. The post holder will be responsible for recording and communicating the results from investigations, uploading data to the data base, and arranging follow up depending on the result of the fibroscan.

The post holder will be responsible for educating patients' on how to reduce their risk of developing further liver damage, such as cutting down on alcohol or eating healthily. They will offer information provided by British Liver Trust.

Most patients with alcohol issues and/or obesity require nutritional interventions to prevent ongoing damage. The post holder will carry out baseline nutritional status assessment, using a validated tool, referring patients to the project dietitian where required.

The post holder may require referring patients urgently to the liver specialist team and / or signposting to support services, such as Change, Grow, Live (CGL)

This post requires an empathetic approach and understanding of barriers these patients face in accessing and engaging in health screening programmes.
