Job summary
We are seeking a highly motivated, enthusiastic and experienced Band 4 Therapy Support Worker to work on the Older Person Assessment Liaison team (OPAL).
Our aim is to reduce avoidable admissions by proactively assessing patients and addressing issues relating to function, mobility and social needs referring on to community services as required. This holistic service means patients receive the support and treatment needed, in the most appropriate setting for them, which will often be their own home supported by community teams.
As part of a multidisciplinary team OPAL provides timely patient centred assessments to those with acute needs within the Heartlands Hospital. We provide effective and efficient multidisciplinary comprehensive geriatric assessment within ED, FSDEC and OPAD.
You work alongside the qualified team members to assist in adopting a home first approach using client centred assessments, expertise and advice to support the patients through their journey.
The area provides early Comprehensive Geriatric Assessments for patients who are 65 and above with the aim to facilitate early discharge and reduce length of stay. The role requires excellent communication skills the ability to discuss highly sensitive matters with patients and relatives in a confidential and sensitive manner.

The OPAL Therapy Support Worker will work under the guidance of the OPAL Clinical Practitioners, Qualified therapist and Assessors. They will gather information from patients or relatives where appropriate, in order for OPAL assessors to carry out their initial assessment and mobility Assessment. They will complete administrative tasks, patient surveys, ordering and booking equipment delivery, making telephone referrals to community beds under the instruction of the OPAL multi-skilled team members. They will complete daily routine checks.

- *Able to demonstrate competence in maths and written and spoken English
