Job summary

The Urgent Community Response Team aims to deliver holistic assessments and interventions to individuals who are at risk of hospital admission within their own home within 2 hours of referral to the service. The aim of the team is to enable people to stay at home, recover, and re-engage with the important aspects of their life and prevent admission to acute sectors.

The post holder will ensure access to Urgent Health Assessments, provide and monitor short term treatment programmes at home and ensure safe discharge or referral to long term health and social care interventions. Utilising clinical expertise, clinical judgement, discretion and decision making in the development of individualised treatment plans.

You will be expected to work multi professionally, support other team members' roles and contribute to the future development of the service providing professional advice and appropriate training to others where required.

To work in partnership with primary, secondary and other health and social care providers, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care.

To put the patient at the centre of all decisions, reinforcing the concept of a more streamlined patient journey, improving the quality of care.

The post holder will be expected to work within a flexible working pattern over a 7 day week in the urgent community response team as required.

Main duties of the job

* Act as an expert practitioner, professional role model and specialist resource to the multi-disciplinary team regarding UCR /Virtual Ward

* Work alongside the Care co-ordination centre to facilitate discharges to community services.

* To work according to the appropriate professional standards, professional practice and the relevant legislative framework and exercise professional accountability always.

* Promote health by collaborative working with patients and their relatives. Act to and facilitate others to respect the dignity, wishes and beliefs of patients; involving them in shared decision making and obtaining consent prior to all interventions.

* Participate in the further development of an integrated, multi-disciplinary Urgent Community Response Service working alongside colleagues in Integrated Multidisciplinary teams and the wider health and social care economy.

* Co-ordinate other members of the Team and appropriately delegate specific aspects of nursing and therapy care according to the care plan

* Participate in and lead clinical meetings to facilitate the continuity of patient care ensuring patients and relatives are kept informed of treatments, progress, and discharge arrangements

* To undertake other duties commensurate with this grade of post in agreement with the relevant line manager

* Completing advance care planning for palliative patients, including respect forms.
