Job summary

We are excited to offer an excellent opportunity for a dynamic and motivated Band 6 Clinical Nurse Specialist to join our innovative Frailty Same Day Emergency Care (FSDEC) team. Based within a fast-paced and forward-thinking environment, the Frailty SDEC provides rapid, comprehensive assessment and care for older adults referred from Emergency Departments, General Practice, Community Teams, West Midlands Ambulance Service, and Urgent Care centres.

As a CNS within our service, you will play a key role in coordinating the clinical area, screening and triaging referrals, and ensuring timely, safe, and effective care is delivered. You will work as part of a multidisciplinary team to deliver holistic, patient-centred assessments and contribute to admission avoidance and improved patient outcomes.

Main duties of the job

Triage and Assess Referrals: Receive, screen, and prioritise referrals from ED, GPs, ambulance services, community teams, and urgent care centres to determine clinical urgency and suitability for SDEC.

Clinical Assessment and Management: Undertake comprehensive, holistic assessments of older adults with frailty, using clinical judgement to support safe decision-making, treatment planning, and appropriate discharge or admission.

Coordinate Daily Clinical Flow: Safely manage and coordinate the daily running of the Frailty SDEC area, ensuring efficient patient flow and effective utilisation of clinic resources.

Collaborative Working: Work closely with a multidisciplinary team including geriatricians, therapists, and pharmacists to deliver high-quality, patient-centred care.

Leadership and Supervision: Provide clinical leadership, support and mentorship to junior staff and students, promoting excellence in frailty care.

Service Development and Quality Improvement: Contribute to service development initiatives, audits, and quality improvement projects to enhance patient experience and clinical outcomes.

Professional Development: Engage in ongoing education and training, including opportunities to complete external clinical courses relevant to frailty, advanced assessment, and decision-making.

Communication and Documentation: Ensure clear, effective communication with patients, families, and healthcare professionals, and maintain accurate, timely clinical records.
