Job summary
We are excited to offer an excellent opportunity for a dynamic and motivated Band 6 Clinical Nurse Specialist to join our innovative Frailty Same Day Emergency Care (FSDEC) team. Based within a fast-paced and forward-thinking environment, the Frailty SDEC provides rapid, comprehensive assessment and care for older adults referred from Emergency Departments, General Practice, Community Teams, West Midlands Ambulance Service, and Urgent Care centres.
As a CNS within our service, you will play a key role in coordinating the clinical area, screening and triaging referrals, and ensuring timely, safe, and effective care is delivered.
