Job summary

An exciting post is now available as part of the renal clinical nurse specialist team.

Following the publication of the White Paper 'Our inheritance, our future -- realising the potential of genetics in the NHS', the West Midlands Regional Clinical Genetics Service secured funding from the Department of Health to support co-ordination of clinical genetics and mainstream adult medicine. This funding facilitated the establishment of a renal genetic service co-ordinating clinical genetics, renal medicine and nurse led genetic counselling. Since the completion of this project the local Primary Care trust has agreed to continue to support the renal genetic service including the funding of one specialist renal genetic nurse post.

The post holder will be based within the renal unit of the Queen Elizabeth Hospital and be a member of the renal and genetic multidisciplinary teams. The post holder will support renal genetic clinics and will liaise with the departments of clinical genetics and paediatric nephrology. The post holder will also support a specialist national multidisciplinary clinic for patients with Bardet Biedle syndrome. The post holder will assist with the development and delivery of a nurse-led service for the management of patients with a family history of genetic renal disease. The post holder will contribute toward the development of a data collection system to support the identification and management of genetic renal disorders.

Main duties of the job

To continue to develop protocols for nurse-led services for patients and their families accessing specialist genetic clinics, newly diagnosed and existing patients with renal genetic disorders.

To continue to develop, and review as appropriate, the patient referral and management pathways in liaison with the project team and the renal clinical leads.

To provide support for the NCG funded national Bardet Biedle Clinic run 6 times per year in the QEH.

To undertake nurse led telephone and face-to-face clinics upon receipt of a referral to undertake genetic and nursing assessment of patient's and their families with specific reference to:

* Detailing the patient's family history ensuring that patients' and their relatives' right to privacy and confidentiality is maintained.

* Confirm diagnostic information

* Interpret medical, family and psychological history

* To create and maintain manual or electronic genetic pedigrees

These functions will include the exercise of diplomacy and tact.

To undertake nurse-led referral on the basis of the patient's family history/pedigree, for those individuals identified as "at risk", to other health professionals/services to advise on inheritance patterns, provide screening recommendations and to recommend a pathway of care. This may include referrals to:

* Other clinical genetic services

* General Practitioners

* Paediatricians

* Local nephrologists

* Specialist treatment centres e.g. Lysosomal storage, Amyloidosis etc.
