TAVI Training Fellowship - Cardiology

Job summary

The 12-month TAVI Training Fellowship at QEHB is aimed at senior trainees in Interventional Cardiology from the United Kingdom, and internationally, who wish to become fully independent TAVI operators. The TAVI programme at University Hospital Birmingham is one of the largest in the country with an annual case load of 300 and rising. The programme commenced in Dec 2008 and the total case number is now over 2500. The team comprises 3 TAVI consultants supported by two Band 7, Advanced Nurse Practitioner and 2 secretaries. The programme switched to a fully percutaneous, conscious sedation programme in Sept 2014.

Main duties of the job

Over 95% of cases are performed via transfemoral access with nurse led sedation. The preferred alternative access is percutaneous axillary access with trans-caval being the second alternative route. Transfemoral procedures are undertaken with a 'minimalist' approach access using micropunture and US guidance, preclosure with Proglides, radial access for the pigtail catheter, rapid pacing via the 0.035" delivery wire and access DSA on completion of access closure.

Patients are recovered to the CCU where over 90% of electives are discharged the following day and with over 80% discharged the next day when urgent cases are included. The centre is an Edwards BenchMark reference centre and a centre of excellence for the Medtronic Evolut platform. In addition the programme has undertaken a large number of valve-in-valve and valve-in-ring procedures for degenerate mitral bioprostheses via trans-septal puncture. In May 2021 a mitral edge-to-edge repair programme was commenced and 90 cases have successfully been undertaken to date using the Pascal device and recently the Mitraclip. The mitral TEER programme is commissioned and a tricuspid TEER programme has also been established. A TTVR programme with the Evoque valve has commenced and a total of 12 procedures have been performed. We will soon commence TS mitral valve replacement with the Edwards M3 platform.
