Job summary

Ward 20A - is a 25 bedded inpatient ward that provides care for patients with complex co morbidities and conditions, together with complex discharge planning. We care for Trauma and Orthopedic rehabilitation patients and those with a variety of long-term conditions, for example, Parkinson's disease, COPD and Dementia, etc. Working closely with various disciplines; Dementia Outreach Team, Social Work Team, Palliative Care Specialist and many others.We are looking for kind, highly motivated individuals with a passion to provide excellent patient centred care to those caring for our patients The individual will need to be motivated and act as a role model delivering high standards of care, supporting new initiatives and continuously striving to acheive outstanding on our ward

Main duties of the job

As a valued member of the nursing team, you will demonstrate a level of skill and understanding which enables you to undertake specific tasks delegated by the nurse in charge and in accordance with relevant procedures, guidelines and policies.

This critical position will work co-operatively as part of the multi-disciplinary team supporting colleagues in work associated with patient care. You will be encouraged to challenge and escalate poor practice/performance.

Use paper and electronic systems to retrieve and record patient information in line with Trust documentation standards and operational systems.

You will communicate clearly, effectively and appropriately with the multidisciplinary team, patients and their family, visitors and carers.

The wards both provide 24 - hour care through a variety of shift patterns. the individual will work closely with the nursing team to carry out observations which include blood pressure, pulse rate, temperature, height and weight, fluid balance, Oxygen saturations, blood sugar glucose monitoring.

Observe and report to the registered nurse all changes in a patient's physical and psychological condition.

you will at times support families and carers following a patient bereavement or when bad news has been shared.

  • *Functional skills Level 2 English Language and Maths Qualification or GCSE Grade C or above in Maths and English Language
