Job summary
We have an exciting opportunity for a Band 7 Community Frailty Practitioner to join the Support to Care Homes Team based in Solihull. The team provides educational and clinical support to staff and residents in Nursing and Residential homes in line with the Enhanced Support to Care Home Framework. The aim of the service is to reduce the number of unnecessary hospital admissions and improve the quality and standards of care. To provide clinical expertise to maintain resident's health and to support and empower care home employees.
The Team is currently collaborating with partners on new initiatives making this role an exciting and forward-thinking place to be. We are looking for initiative-taking individuals with excellent communication skills and a passion for improving the quality of care. The ideal candidate will be innovative and invested in all aspects of older adult care and be a collaborator in the promotion of quality and standards in line with patients' needs and preferences.
You will work in partnership with primary care to provide expert clinical case management for patients with complex care needs and frailty who are at risk of deteriorating health that may result in a decline in their quality of life. This includes assessing, diagnosing, prescribing, and implementing a range of core interventions with an emphasis on avoiding inappropriate hospital admission or an unnecessary longer length of hospitalisation. A teaching and assessing qualification is recommended.
Main duties of the job
Community experience at Band 6 is desirable. Communication skills must be of a high standard along with well-developed interpersonal skills, which facilitates relationships with patients, carers and MDT
Applicants must be able to demonstrate clinical judgement and decision making, assessment, diagnosis and planning of individual care needs. You will work in partnership with patients, families, and MDT to deliver evidence-based care that is both proactive and responsive to your patient's needs. You will work as a key practitioner in the co-ordination of patient care, therefore the ability to identify where an MDT approach is needed is essential
You will have experience of effective caseload management, leadership and change management. You will work as part of a busy team and must be able to manage your time effectively, prioritise your workload and be able to work under pressure to meet deadlines and time restraints.
Data analysis aids the team to support care homes clinically and educationally, the post holder will be responsible for analysing and interpreting data. Candidates should also have some experience in designing and delivering teaching packages. Experience of developing clinical guidelines, policies and integrated care plans is an advantage. An interest in improving quality is essential for this role and candidates must be willing to undertake clinical audits
Ability to travel across the Solihull borough and therefore a car driver/owner is essential for this role

- *Non-medical prescriber or willing to work towards ( where applicable to role )
- *Evidence and ability to revalidate as required by the NMC.
