A rare and exciting opportunity has come up for two Therapy Support Workers in the Solihull Community Therapy and Specialist Falls Teams.
The role of the Band 4 Therapy Support Worker is to support the delivery of community-based assessments and rehabilitation to housebound individuals with a Solihull GP.
The role involves working in a Multidisciplinary team of Occupational Therapists, Physiotherapists, Nurses, a pharmacist and therapy support workers.
This role will involve levels of autonomy with overall supervision provided by a Team Lead and clinical supervisor, you will be responsible for an agreed specific caseload and will manage patients through their therapy journey with the team.
Alongside this you will be responsible for supporting all members of the team with admin related tasks to include managing telephone calls, monitoring and booking into staff ledgers, letter writing and triage, to ensure the services runs smoothly and efficiently.
Both services operate a 7-day working rota (weekend working) which the individual successfully securing the post will be expected to participate in. This post supports the teams 7 day working rota (weekend working) and requires a person with a full driving licence and access to a car with business insurance.
Main duties of the job
The role of the Band 4 Therapy Support Worker is to support the delivery of community-based assessments and rehabilitation to housebound individuals with a Solihull GP and to support the team with smooth running and efficient working processes. We are looking for 2 experienced individuals to deliver the role requirements across the Solihull Community Therapy Team and the Solihull Specialist Falls Service. These roles are based on site at Solihull Hospital with some expected travel to provide domiciliary (home based) rehabilitation visits to patients within their own homes and care homes across Solihull. Both posts will also include administrative duties to support the services relating to day to day running, protocol-based triage, case management and patient flow.
The role involves working within a busy multidisciplinary team including registered Occupational Therapists, Physiotherapists and band 3 and 4 Support workers. These Community based services receive over 300 new referrals each month for patients (who are often older in age) and who may have been in hospital or who are in the community but require therapeutic specialist input.
The role will involve levels of autonomy with overall supervision provided by a qualified staff member. You will be responsible for managing an agreed specific clinical caseload to support patients through their therapy journey within the team.
