Job summary
Liver disease and liver damage are often diagnosed at a very late stage when there is little hope of improving the patients' health. Chronic liver injury can lead to various health problems including liver cancer. Diagnosing liver disease early and treating the causes should improve health outcomes.
Funded by NHS England / NHS Improvement and Birmingham City Council, the project aims to screen for liver disease in at risk populations and for those individuals with cirrhosis, engage and retain them in the liver cancer surveillance pathways (6 monthly USS / bloods and clinical review). Ensuring all eligible patients are included on liver surveillance registers.
This post is a new role to support the delivery of the Community Liver Health Checks pilot (CLHC) programme at the UHB and across the West Midlands. The post holder will coordinate the care and monitor surveillance pathways for patients with cirrhosis working with patient navigators and peer support workers to improve patient outcomes
Main duties of the job
This is an exciting opportunity for an enthusiastic, organised and well-motivated individual to lead the pilot delivery team, based at UHB, to identify patients in West Midlands area who are at risk of developing liver fibrosis or cirrhosis. Those found to have liver cirrhosis will be supported to engage in liver surveillance pathways. The post holder will work according to the national protocol and contribute to developing national standards and optimise liver surveillance pathways locally.
The post holder will use their clinical skills to assess qualifying patients for evidence of liver damage using several testing methods for diagnosis, such as taking blood tests and fibroscans. The post holder will be responsible for recording and communicating the results from investigations, uploading data to the data base, and arranging follow up depending on the result of the fibroscan.
The post holder should have excellent communication skills to co-ordinate patient care amongst many professionals.
This post requires an empathetic approach and understanding of barriers these patients face in accessing and engaging in health screening programmes
