Job summary
To deliver early access to diagnosis and appropriate management of valve disease by a consultant physiologist-led in-patient and out-patient service, supported by guideline-based direct access to valve intervention for severe left heart valve disease and to valve clinic review for moderate left heart valve disease.
All patients with asymptomatic or symptomatic suspected moderate or severe left heart valve disease would be referred to a single point of access. Patients with known left heart valve disease, prior valve intervention and congenital heart disease would be excluded. Pregnant patients would also be excluded from the service.
Set up direct access to a single point of referral, Consultant Physiologist-led screening within two weeks. The referral would lead to an appointment comprising clinical assessment, examination and one-stop transthoracic echocardiogram delivered by a physiologist trained in heart valve disease diagnosis, examination and management. This should include using the CDC's available to the trust to minimise patient travel and maximise the footprint of patients available to register.
Main duties of the job
The primary outcome would be time to guideline-directed management of heart valve disease, compared to standard referral pathways through the Cardiology department. Secondary outcomes will be collected, including time from referral to decision on management, patient reported measures of satisfaction, and safety outcomes (including requirement for urgent or emergency intervention and heart failure hospitalisation).
If supported in the application, health economic assessment of the proposed model would be included, compared to standard clinical pathways.
Patients with a positive finding with moderate and severe valve disease will have access to clinic slots made available by consultants for a review to early intervention. The vision would also include taking straightforward cases direct to MDT, following completion of relevant, required investigations. The post-holder will need to be present at MDT and present cases.
