Band 6 Nurse Specialist for Children in Care

Job summary
The post holder will be based in Solihull with a defined caseload of children and young people in care.
The post holder will work autonomously and in collaboration with colleagues across the children's workforce to demonstrate improvements in health and wellbeing outcomes of children looked after by Solihull Local authority and those placed in Solihull by other local authorities.
The post holder will assess, plan and evaluate the health needs of children on their caseload, challenging and escalating concerns where needed to ensure that the needs of the child are paramount.
The post holder will ensure that the voice of the child clearly informs the delivery of services and their own practice
*Work with the wider team to continually improve the service through training, supervision, quality assurance and audit to ensure the consistent delivery of high-quality services.
*Be required to work flexibly across the Trust to meet the needs of the service.
*Contribute to the monitoring and improvement of the service performance (against local and national targets) for CYPLA under the care of Solihull Local authority
Main duties of the job
*Implement national and local standards of care in accordance with legislation and local policy.
*To proactively work with all Solihull CYPLA and those placed in Solihull by other local authorities (where requested).
*Recognise the diversity of CYPLA and ensure care planning accommodates individual need.
*Support development of care pathways
*Collate the views of CYPLA regarding their experience of care and their health needs. Consider any gaps and how these can be met in the multi- agency arena.
*Delivery of high quality and timely statutory Review Health Assessments for CYPLA using evidence to inform practice. Be responsible for assessing, planning and implementing flexible programmes of care or referral onto other services where appropriate.
*To compile health summaries for care leavers and support effective transitions for care leavers including those with complex needs who also may require care packages into adulthood.
*To provide when required complex information across a range of agencies, professionals and carers within and outside of the borough.
*Collaborate with health and social care colleagues, and other statutory and voluntary agencies, to ensure all children have a smooth transition from pre-school to school age and through to leaving care and independence.
*Plan and deliver training for health professionals, Social Workers and Foster Carers as required.
