An exciting position has arisen for a proactive and dynamic individual with an appropriate level of knowledge and skills to join the Pharmacy team, leading on Neurosciences at UHB. The post holder will also work closely with our Specialist Pharmacists for Epilepsy and MS as well as the pharmacy operational team on site to ensure day to day provision of the clinical pharmacy service to neuroscience wards.
The post holder will lead & coordinate the development, delivery & evaluation of pharmacy neuroscience services. The post holder will further develop, deliver and evaluate the service to support the medicines optimisation agenda.
The Trust has close links with local universities, you will be involved with supporting the undergraduate pharmacy teaching. You will also contribute to the delivery of formal and informal education and development programmes, including the Diploma in Clinical Pharmacy, and our rotational programme for band 6 and 7 clinical pharmacists.
As part of this job, you will be expected to work as part of a wider team, but also independently and must possess effective influencing and negotiating skills.
The post holder may be requested to work cross-site with the other lead pharmacists in the team.
Main duties of the job
*To maintain a general awareness of medication practice on wards and to identify where practice goes outside safe limits. To enforce good practice and apply the guidelines in the Trust Medicines Management Policies including the Medicines Policy.
*To support the specialty to write medication-based treatment guidelines and clinical protocols, patient group directions, standard operating procedures, and other documents.
*To assure that the policies and procedures relating to the administration of medicines are in accordance with national Standards.
*To give advice on medicine dosage, method of administration and side effects which may be anticipated together with suitable preventative measures.
*To contribute to the development of risk management and clinical governance strategies within the speciality.
*To screen prescriptions and identify anomalies such as interactions, contraindications, inappropriate doses, failures to respond to pathology results. To influence prescribers to change their drug choice or rectify an error.
*To clarify prescription ambiguities so that staff dispensing the medication and staff administering the dose will not be misled. These activities will take place daily on allocated wards and (on rotation) in the clinical checking centre in the dispensary. In some cases, similar duties are carried out on visits to external units.
