Job summary

The Alcohol Care Team Practitioner will be an integral member of the Alcohol Care Team, which provides care and treatment for patients with alcohol misuse issues in a hospital environment.

To work collaboratively with the Alcohol Care Team and wider multidisciplinary teams in order to support and assist the patients' health care journey, the Alcohol Practitioner will assess the needs of the individual patient and facilitate the appropriate channels of support.

To provide support to the Alcohol Care Teams across three sites, Queen Elizabeth Hospital, Heartland Hospital and Good Hope Hospital. Working in conjunction with colleagues, service users and stakeholders to provide an efficient service.

It is essential that the post holder exercises initiative and judgement using acquired skills and knowledge, always maintaining confidentiality. The post holder will provide an empathic and sensitive point of contact for service users and to signpost into other services and integrate care with other providers of health services. It is essential for the post holder to develop strong professional relationships with colleagues, Trust employees, and staff from external agencies.

The Alcohol Practitioner will be flexible to meet the needs of the team and will demonstrate knowledge, expertise and passion in working within the speciality of alcohol misuse.

The Alcohol Care Team Practitioner will provide education and support on the role of Alcohol Care Team to internal staff and external teams to UHB.

Main duties of the job

*Build partnerships and therapeutic relationships through safe, effective and non-discriminatory communication taking account of differences, capability and needs of individuals.

*Communicate with patients and their relatives/carers, making reports and liaising as required with medical staff and other members of the multidisciplinary team both verbally and in writing. Ensure instructions/information is understood and if required acted upon. Be mindful of own and others body language and barriers which may affect communication.

*To support other members in the Alcohol Team in the care and management of intoxicated patients and patients in acute alcohol withdrawal.

*To develop strong professional relationships with colleagues, Trust employees, Trust volunteers and staff from external agencies.

*Maintain a high standard of professional behaviour and ensure effective and timely communication with all members of the multidisciplinary team, patients, carers and relatives.

*To assist with the collection and provision of statistical data as required and the ability to extract, manipulate, analyse data sets and communicate them in an easy to understand manner

*To screen patients to identify those at risk from alcohol related health and social problems and those whose health is being damaged by alcohol.

*To make appropriate referrals into community services and to signpost into other services and integrate care with other providers of health services
