Job summary

In partnership with the Birmingham Community Health Care Trust and the advanced heart failure programme at the Queen Elizabeth Hospital have created a new exciting Band 7 CNS HF/CR Nurse Specialist role. The purpose of this role is to support the implementation and evaluation of a seamless specialist cardiac rehabilitation service, focused on clients with heart failure and/or respiratory disease. Central to the success of this innovative role will be providing on-going support of heart failure patients recruited to the programme that promotes their engagement in the exercise and education classes. In addition, the role will be promoting re-engagement where indicated, medication compliance and escalation to appropriate health care professionals where symptom review or deterioration is suspected so that acute presentation to hospital can be avoided.

Working with members of both organisations, the post holder will, be hosted and managed by UHB/QEB. Through direct involvement, support and contribute to the content and delivery of the going education, exercise, in this innovative community cardiac rehabilitation programme, organised and managed by Birmingham Community Health Care Trust (BCHC). Supervision will also be provided by BCHC when working in their facilities.

Essential to the role is excellent written and communication skills and the ability to work both autonomously and within the broader heart failure/co morbidity MDTs.

Main duties of the job

* To act as an expert practitioner working autonomously, managing a caseload of patients within a speciality whilst working as part of a multidisciplinary team (defined or virtual), delivering individualised and personalised direct patient care.

* Provide a specialist nursing service and advice in a variety of clinical settings and disciplines for staff, patients, their families and carers, effectively liaising with multidisciplinary colleagues. Triage case load according to agreed protocols and working practices.

* Participate in nurse led and multidisciplinary clinics, ward rounds, patient reviews and multidisciplinary team meetings, where the post holder will co-ordinate treatment independently or in conjunction with a wider multidisciplinary team. Undertake and provide treatment / advice as per speciality and scope of practice. These may be face to face, virtual or telephone, and incorporate the use of agreed protocols clinical and professional guidelines. This service may be provided within an acute secondary health care facility or in primary care or home care settings in accordance with local service level agreements.
