Job summary

To act as an autonomous practitioner working independently and in conjunction with other health care professionals to assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate.

The ANP will be able to independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.

The ANP will work collaboratively with the general practice team, to meet the needs of patients and support the delivery of policy and procedures, and provide nursing leadership.

Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence.

The ANP will co-ordinate all available nursing resource, working with the practice manager and GPs to deliver the practice priorities.

To develop new and innovative concepts, models, methods and practices to deliver new and improved care services to meet the needs of service users.

To provide education and training to other staff and students.

To undertake research as part of the role.

Main duties of the job

* Prioritise and triage patients presenting with a range of acute and chronic conditions,

making any necessary referrals for investigations in the appropriate manner.

* Implement and evaluate individual specialised treatment plans for patients.

* Provide advanced, specialist nursing care to patients as required in accordance with clinical

based evidence, NICE and the NSF.

* Provide specialist clinics as required.

* Undertake the collection of pathological specimens/request pathology services as necessary.

* Process and interpret pathology and other test results as required.

* Maintain accurate clinical records in conjunction with extant legislation.

* Ensure read codes are used effectively.

* Chaperone patients where necessary.

* Prioritise health issues and intervene appropriately.

* Support the team in dealing with clinical emergencies.

* Recognise, assess and refer patients presenting with mental health needs.

* Be an extended and supplementary prescriber, adhering to extant guidance.

* Support patients in the use of their prescribed medicines or over the counter medicines

(within own scope of practice), reviewing as required
