Job summary

This is a fantastic opportunity for Occupational therapists to progress in their careers.

Our team, The Solihull Community Services Intermediate Care, operates out of multiple locations in Solihull. We are expanding our therapy team to offer services to more individuals who require further assessment and rehabilitation after being placed in discharge-to-assess beds and the patient's own home for further rehabilitation. We focus on those with complex discharge planning needs following a hospital admission.

Our team is dedicated to fostering a positive and progressive culture that prioritises the ongoing professional and career growth of our staff. In keeping with the discharge-to-assess model, we are constantly refining and enhancing our service, which presents exciting prospects for involvement in its expansion and improvement.

Our team is dedicated to delivering a comprehensive 7-day therapy service, which entails being part of a weekend coverage schedule.

This opportunity may be ideal for an experienced Band 5 professional seeking career advancement or a current Band 6 professional interested in collaborating with a team to facilitate ongoing assessment and treatment progression of patients with complex discharges from acute hospitals.

Driving and access to a vehicle is essential for this role.

Main duties of the job

*To deliver a high standard of therapy provision within the Solihull Community and Intermediate Care Bedded Units.

*To support our patients in their recovery journey and minimise the risk of hospital readmissions.

*To make the transition from acute care to the community as smooth as possible, ensuring timely discharges and efficient flow.

*To liaise with various agencies to ensure safe, effective and efficient discharges.

*To provide OT services in the patient's own home or residence.

*To contribute to seven-day working programmes across the organisation.

*To be able to manage your own caseload and work independently.

*To be part of the flow process (Triaging and processing of the referrals) as a clinician and also be able to manage associated admin processes when required.

*To supervise students on placements.

*To appraise, supervise and support junior staff and therapy assistants.

*To initiate/participate in audits, research and quality improvement programmes.
