Job summary
We are pleased to offer this exciting opportunity to join our friendly and dynamic team on Ward 408.
Ward 408 is a very busy, multispecialty ward with a quick turn-over rate. We are looking for a highly motivated and organised individual with excellent communication skills, who will act as an integral part of the team. They will also be providing Flow Co-ordinator services for The Burns Unit - a department that the ward works closely with.
The post holder will provide a comprehensive and full ranging administration/Co-ordination service, working in conjunction with colleagues, service users and stakeholders to provide an efficient service.
The post holder will play a pivotal role in both tracking patients through their clinical pathway ensuring the waiting time targets/ admission and discharge or transfer of care processes and pathways are being met, recording relevant data along the patient's journey. With a focus on urgent and emergency access and inpatient stay, the post holder will work closely with the clinical and operational management teams, ensuring all patients referred to and cared for by the speciality are tracked and have treatment and management plans and that there are effective discharge or transfer plans in place.
The post holder will exercise initiative and judgement using acquired skills and knowledge, maintaining confidentiality at all times. They will provide an empathic and sensitive point of contact for service users.
Main duties of the job
*Collaboratively communicating and working with clinical/operational teams both within and outside of the trust to ensure appropriate management of patient information and co-ordination of patient pathways.
*Negotiate and improve the co-ordination of the patient journey.
*Support the nurse in charge of departments /ward to monitor and ensure the most effective use of beds within the service. Monitor appropriate admissions and in patients progress on a daily basis and prevent discharge delays wherever possible. Escalate any delays in patient flow to clinical and operational teams.
*Organise own day to day work and prioritise team tasks as required.
*Facilitate the provisions of information and co-ordinate the function of the multi-disciplinary team meetings, board and ward rounds , by ensuring lists of patients to be discussed and other relevant information are prepared and where possible distributed in advance of a meeting.
*Collate any necessary information as requested in relation to the referral/MDT to ensure supporting actions are documented in agreed methods. Report information collated either electronically, verbally or in writing on request.
*Provide a point of contact for staff and service users: accurately record telephone messages and take action as appropriate.
*Be sympathetic to the requirements of staff, service users when communicating and be mindful that some communication may be difficult to convey and distressing to receive.
