Job summary

We are seeking a motivated and compassionate Practice Nurse to join our multidisciplinary team. You will play a key role in delivering high-quality nursing care to our patient population, supporting both chronic disease management and preventative healthcare.

We are looking for a nurse who will primarily support patients with diabetes whilst also getting the opportunity to utilise their broader nursing skills in other areas.

This post is for a fixed 6 month term, working 18 hours per week.

Interviews to be held potentially week commencing 20th April 2026.

Main duties of the job

Deliver general nursing services including wound care, immunisations, cervical cytology, and health checks

Support the management of long-term conditions such as diabetes, asthma, and COPD

Provide health promotion and lifestyle advice to patients

Assist in infection prevention and control measures

Maintain accurate clinical records in line with practice policies

Work collaboratively with GPs and the wider healthcare team

About us

Our practice is run by Heartbeat Primary Care, a Community Interest Company which also supports 16 GP practices across Hambleton and Richmondshire.

We are a PMS practice with a list size of around 8,400 patients based in Catterick Garrison, North Yorkshire. Catterick is the British Armys largest Garrison in the world and is set to double in size by 2030 with the expectation that our registered list will also grow proportionately over the next 8 years.

A new integrated care centre is currently being built to meet the requirements of this projected population growth and is likely to form the blueprint for similar facilities in communities with a combined defence and civilian population. We have a history of innovative service development to meet the needs of our population, which consists mostly of the families of serving soldiers and second-generation army families who have settled in the area.

We are a well-established training practice, providing continued development opportunities to our staff and trainees while working as part of one united PCN which provides important benefits to the individual practices, the wider group and most importantly our patients.

Job description

Job responsibilities

KEY ROLE CLINICAL NURSING PRACTICE

Undertakes procedures to include, venepuncture, BP management, urinalysis, new patient health checks, pregnancy testing, stitch/clip removal, wound management, ECGs, cervical screenings, contreception advice and administration.

Administers adult and child immunisations and vaccinations in accordance with national and local programmes. Is competent in anaphylaxis and resuscitation techniques. Ensures safe storage, rotation and disposal of vaccines and drugs and adequate stock levels are maintained.

Assists GPs with minor surgery procedures, coil fittings etc

Recognises and understands the importance of observing, documenting and communicating the signs and symptoms of child and adult abuse.

Works within all relevant practice policies and procedural guidelines eg infection control, chaperoning and patient group directions.

Work within NMC Code of Conduct and practice policies.

KEY ROLE CARE AND PROGRAMME MANAGEMENT

Takes a lead in specialist clinical areas within own competencies (e.g. CHD, family planning etc) and undertakes the establishment and running of disease management clinics providing specialist nursing care for the patients and communities that the practice serves.

Initiates the process of diagnosis with patients suspected to have a chronic disease e.g. diabetes, COPD, asthma, CHD referring to other clinicians as appropriate.

Manages programmes of care for patients with long term conditions/chronic diseases by planning, providing and evaluating care under agreed local/national guidelines, protocols and patient group directions.

Maintains disease registers.

Audits outcomes of care against standards and initiates changes as necessary.

Undertakes annual review of patients understanding and ability to self manage.

Refers to other members of the extended primary health care team eg chiropodist, dietician, specialist nurses.

KEY ROLE HEALTH PROMOTION

Initiates and carries out programmes of health screening.

Provide holistic, patient-centred care.

Contributes to profiling of the practice population.

Enables, encourages and supports individuals, families and groups to address issues which affect their health and social well-being.

Provides healthy lifestyle advice to individuals.

Support patients in understanding and managing their conditions.

Act as an advocate for patients where appropriate.

Develops and maintains the health promotion material within the practice.

Makes referrals to statutory and voluntary agencies as appropriate.

KEY ROLE LEAD SELF/OTHERS

Contributes to and initiates the development, monitoring and evaluation of care delivery by the Nursing Team.

Promotes effective communication and relationships within the team.

Works collaboratively as a team member to achieve the objectives of the PMS contract and aspirations of the team.

Provides clinical advice on educational opportunities that will facilitate the development and support of their specialist knowledge and skills to ensure the development of clinical practice.

Partakes in clinical supervision for own development.

Actively participates in clinical and practice meetings.

Develops own knowledge and practice to meet objectives/changes in service.

KEY ROLE MANAGE RESOURCES

Monitors and uses resources appropriately within the team eg dressings, vaccines, stationary.

Ensure stock control of vaccines and medications.

Makes effective use of the clinical system by ensuring the consistent and accurate entering of data and use of Read Codes.

Contributes to a satisfactory call and recall system of patients
