Job summary

An exciting new opportunity has arisen for us to recruit a Part Time (30 hours per week) Clinician to join our rapidly expanding primary care team, working across Richmondshire Primary Care Network. The Network comprises of 8 practices covering Richmond, Leyburn, Hawes, Catterick, Scorton and Aldbrough St John and supports almost 44,000 patients across the area.

We are looking for an experienced and motivated Clinician to join our existing team to support our GPs and the wider primary care workforce. Working within your professional boundaries, you will be responsible for working collaboratively with the multi-disciplinary primary care teams across all practices within the PCN.

One of the main projects of the PCN has been the creation of a specialist visiting team to coordinate the provision of tailored and multi-disciplinary care to residents in Care Homes and Supported Living settings in order to improve their planned care experience. This meets the requirements of the national Enhanced Health in Care Homes Service Specification. This role will be an integral part of the team and have the opportunity to be influential on how their role will evolve as the service develops.

You will add a unique and valuable contribution to the skill mix of the primary care team and multi disciplinary working within Care Homes and Supported Living Accommodation.

Main duties of the job

You will be required to work as an integral senior member of a multi-disciplinary team providing a seamless range of services:

  • Weekly contact visit to each of the 6 care homes or telephone contact or video
  • Identification of residents of concern for preventative/proactive admission avoidance
  • Holistic assessments supported by Community Primary Care Team to assess, plan and deliver specific interventions for patients in care home and supportive living settings
  • To continually monitor, evaluate and modify treatments and care plans as appropriate
  • Manage a caseload of complex clients using advanced problem solving skills and advanced clinical reasoning skills
  • Medication support/Prescribing/Vaccination
  • Proactive care planning including DNACPRs, Advanced Care Plans and Emergency Care Planning to improve care and reduce avoidable admissions
  • To maintain patient records and documentation which conform to legal and service standards
  • Referrals to other specialist services and teams
  • Signposting and staff/family support and education
  • To support ways in which family and patient views can be sought and inform service delivery and care plans
  • Maximise opportunities for Health Improvement
  • Liaising with Lead GP of Care Home /Community Services/Secondary Care /families/NOK where required
  • Telephone triage and support coordination and follow up to other specialist services
  • Routine (non urgent) follow ups and identify themes of Emergency Attendance

About us

You will be employed by our local GP Federation, Heartbeat Primary Care CIC. It represents twenty-one practices across the localities of Hambleton, Richmondshire and Whitby and was formed in 2014 to facilitate and encourage joint working between practices and with other partners. It has forged close working relationships with all member practices across the locality and continues to support the development of relationships across primary care. It has an outstanding track record of collaboration and innovation and currently hosts several PCN Additional Roles posts across the locality.

You will have monthly clinical supervision meetings with other members of the team and a GP, as well as monthly team meetings with the PCN Clinical Director and the Digital and Transformation Lead.

Job responsibilities

Our Care Home Liaison Team is a multi-disciplinary team based a Scorton Medical Centre, Scorton, Richmondshire and provides support to our aligned Care Homes, our proactive frailty support and acute visiting service to housebound patients.

You will provide care for Richmondshire's care homes and supported living populations, from initial assessment of a new resident, review of residents who have recently been discharged from hospital, review of stable residents and those who are deteriorating and approaching end of life care. You will be able to demonstrate critical thinking in the clinical decision-making process, including assessment and diagnostic skills, leading to the delivery of safe and appropriate care for all patients. Working collaboratively with the care home visiting team to meet the needs of the patients whilst supporting the delivery of policy and procedures.

You will provide a holistic and clinical service, with support from GPs and other team members as required, implementing agreed management plans. You will review and act appropriately on blood results and referrals/correspondence regarding patients.

The post holder will work with the wider PCN team to ensure that all aspects of the Primary Care Network Contract Directed Enhanced Service and its associated service specifications, relevant to this post, are realised.

The role is pivotal to improving quality of care for patients and operational efficiencies within primary care and therefore requires motivation and passion to deliver excellent service within general practice.

Key responsibilities

  • Plan and manage complete episodes of care, working in partnership with other members of the multi-disciplinary PCN team, delegating and referring as appropriate, to optimise health outcomes for patients
  • Able to use an advanced level of professional accountability, autonomy and judgement for seeing, treating and assessing patients who present with a broad range of situations and conditions
  • Work autonomously, clinically examining and assessing patients and initiating any appropriate action whilst taking responsibility for managing practice whilst using an advanced level of professional accountability, autonomy and judgement for highly complex decision making across a broad range of situations
  • Have a level of understanding of PCN and NHS values, policies and procedures
  • Provide support to Care Home staff, which may include delivering training where required or supporting Care Home pilots and support the other members of the Care Home Liaison Team
