The Falls and Frailty Prevention Team works across the Great Yarmouth and Waveney area supporting service users registered with a Great Yarmouth and Waveney GP. The Falls and Frailty Prevention Team works Monday to Friday 08.30 to 16.30 and consists of Physiotherapists, Occupational Therapists and Assistant Practitioners working together with the Primary Care Home teams in provision of the service.
Main duties of the job
An exciting opportunity has arisen for an enthusiastic and motivated Assistant Practitioner to join the team based at Yare House, Great Yarmouth. The role consists of a range of office and mobile working. ECCH have electronic patient records and there is an expectation that you will be a mobile worker using a laptop, you will receive support and training to enable you to use this functionality. Full in house and competence based training will be provided for you to fulfill your role.
This role offers opportunity for independent and team working utilizing skills and knowledge to support patients who have experienced a fall and are living with frailty. Working together we aim to achieve patient specific goals, maintain a level of independence and to prevent unnecessary admission to hospital.
The team accepts referrals from Primary Care, Secondary Care, Ambulance service and community colleagues for patients who have fallen or are identified as at risk of falling due to frailty.
The Team will endeavour to advise patients, relatives and carers on how best to assist patients to remain well and manage their everyday living to maximise independence and whenever possible to remain at home. Our holistic assessments include reviewing the home environment and identifying factors which may contribute to falls, where able mitigating the risk and enabling patients to remain safely in their place of residence.
