Leg Ulcer Lead - Sister/Charge Nurse

Job summary

An exciting opportunity has arisen for two Sister/ Charge Nurses (1 North, 1 South) to join our Primary Care Home team to oversee the management of the Leg Ulcer Care, focusing on the care and treatment of individuals with lower limb wounds in the clinic setting and their own homes. If you have a passion for managing leg ulcers and are motivated, adaptable, forward thinking and see yourself leading in this area within community healthcare, then this could be the role for you.

You will be responsible for overseeing Leg Ulcer clinics, patients in the community and also the Leg Ulcer CQUIN , ensuring that there is an equity of care and that individuals are seen in a timely fashion and receive the appropriate treatment whether in the clinic or at home. You will also use your skill and knowledge to support the team and other professionals to become competent and confident in managing lower limb wounds, including referring on when appropriate. You will provide specialist/ generalist training sessions for staff across Great Yarmouth and Waveney localities.

You will have some line management responsibility so will have the opportunity to participate in HR processes including recruitment, appraisals and supporting junior members of the team.

The core nursing shift is 08:30- 16:30 Monday-Friday. The post is offered up at 37.5 hours per week, working 7.5 hour shifts over 5 working days. Flexible working patterns would be considered.

Main duties of the job

To provide clinical leadership, operational management and professional support to community nursing within Primary Care home.

To work with other members of the multidisciplinary team to provide planned and unplanned care.

To provide expert clinical case management for patients living in the community who require planned and unplanned nursing care, either at home or in a clinic setting.

To work autonomously at a level appropriate to clinical competence and within scope of professional practice.

To carry out leg ulcer assessments and treatment in patients own home and in a clinic setting with the aim of providing gold standard care. These assessments will take a holistic ethos including physical, physiological and social aspects of need.

To oversee the management of the Leg Ulcer Clinics.

The post holder will be an autonomous practitioner that is responsible for own case load and to take a lead in the delivery of projects to improve service delivery, patient safety and quality of care.

To provide specialist/ generalist training sessions for staff across Great Yarmouth and Waveney localities.

Contribute to the continuous evaluation of the service to ensure it is fit for purpose and increases choice through audit and research.

Contribute to recruitment and retention of staff and team development within Primary Care Home.
