We are seeking a Registered Nurse (Adult) with experience of working in a community clinical role at a supervisory/ management level. Specialist Practitioner District Nursing Qualification is desirable.
This post is responsible for the professional nursing leadership within the PCH, and leadership for the Out of Hours nursing service, ensuring a high standard of nursing care is maintained, with specific responsibility for the oversight and professional relationship with care homes in the area. The successful candidate will work in close collaboration with the other PCH Nurse leads on service wide nursing developments, and with other Gorleston PCH Leadership roles to ensure the safe delivery of the service.
The PCH is a multidisciplinary team delivering health care to people in the coastal town of Gorleston. The combined skills of the team mean they are able to respond to the changing needs of patients and support where appropriate to remain at home. You will become an active member within this team of clinicians.
The PCH team provides community services 7 days per week 07:00- 20:30 hours
The core shift is 08:30- 16:30. Weekend and bank holiday working on a rotational basis as part of the Band 7 leadership role. The post is offered up to 37.5 hours per week. We are able to support flexible working arrangements.
Main duties of the job
As core member of the PCH leadership team ensure effective staff coordination, case management and service development to deliver high quality nursing care to patients on the planned and unplanned caseload.
Develop and maintain clinical skills in line with the roles competency matrix and specific professional registration.
Provide clinical leadership, operational management, and professional support to registered nurses, Registered Nurse Associates, Assistant Practitioners, and Health Care Assistants within the PCH.
Lead and provide oversight of PCH relationship and support to residential care homes.
Provide nursing support and advice to other members of the PCH team on the care of the deteriorating patient.
Work in partnership with other senior members of primary care home to ensure effective delivery of planned and unplanned services through multidisciplinary team working.
Provide clinical leadership, operational management, and professional support to other members of the multidisciplinary team within the PCH.
Provide leadership and management to the Out of Hours Nursing Service.
To engage with recruitment and retention of staff, skill mix and team development.
Participate in the PCH Band 7 7-day leadership
