EXCITING OPPORTUNITY, NOT TO BE MISSED! The Great Yarmouth & Northern Villages PCH are looking for a dedicated Physiotherapist to join their busy team, which covers Great Yarmouth & the surrounding villages.

ECCH is looking to recruit community physiotherapists to join our successful Primary Care Home teams. The successful candidate will have a passion for community working, excellent communication skills and the ability to prioritise and manage their time effectively. Previous community experience is desirable but not essential, as full training will be provided.

Our Primary Care Homes are multi-disciplinary teams including Nurses, Community Matrons, Paramedics, Occupational Therapists, Physiotherapists, Assistant Practitioners, Rehabilitation Support Workers and Social Workers. We work with patients to help them maintain their independence and live well at home, avoiding the need for hospital admission wherever possible. The service can also provide intensive short-term support (up to 2 weeks) following discharge from hospital. We are a friendly and dynamic team who are proud to work together to provide an excellent service for our patients.

Main duties of the job

To provide a high standard of physiotherapy in the specialist areas of Primary Care Homes, within the community and domiciliary settings throughout ECCH CIC.

As the physiotherapist within the team, you will complete assessments of patients on the reactive caseload, agree goals and set up rehabilitation programmes to be completed by our highly skilled Rehab Support Workers. You will review and progress these plans to maximise the rehabilitation potential of your patients. You will be involved in equipment provision and will advise on moving and handling plans. In addition you will carry a caseload of more routine community patients, for example those who may be at risk of falls or require longer term rehabilitation. You will also have the opportunity to develop your supervision skills, providing support and training to other members of the team including Assistant Practitioners and Rehabilitation Support Workers.

  • To maintain own clinical professional development (CPD) by keeping abreast of any new trends and developments and incorporate them as necessary into your work.
  • To be an active member of the in-service training programme by attendance at training sessions in-house, tutorials and by attending external courses and practicing reflective practice.
  • To maintain accurate, comprehensive and up to date documentation in line with legal, departmental and ECCH requirements, communicating assessment and treatment results to the appropriate disciplines in the form of reports and letters.
  • To participate in staff appraisal scheme and personal development plan (PDP).
  • To undertake the measurement and evaluation of your work and current practices through the use of evidence based practice (EBP) projects, audit and outcome measures, either individually or with other clinical staff and the clinical lead for physiotherapy. Make recommendations for change.
