Senior Occupational Therapist - Intermediate Care

Job responsibilities

Patient Care

Accountable for care standards and the delivery of care promoting a culture of continuous improvement of clinical standards with particular emphasis on patient experience and key clinical outcomes. Involve patient in the planning of care wherever possible, and act in the best interests of the patients at all times. Actively manage the patients admission, involving clinical experts, when relevant, to advise in the diagnosis and treatment. Proactive discharge management, ensuring patients are consulted and proper management of situations where patients need assistance in decision making.

The post holder will adopt a case management approach to ensure that rehabilitation reablement is actioned and integration of care takes place.

To assess, plan, implement and evaluate care and treatment, ensuring the highest quality of patient centred, goal directed care at all times, and with appropriate involvement and support of relatives and carers

To interpret and analyze clinical and non-clinical facts to form an accurate diagnosis and prognosis in a wide range of conditions, to recommend the best course of intervention and to develop comprehensive management strategies for the patients presenting condition

To enhance and develop the skills and dexterity, co-ordination and palpatory senses for assessment and manual treatment of patients. To be safe and competent in the use of therapy equipment.

To refine treatment techniques in line with the most recent evidence of clinical effectiveness by evaluating and implementing key developments and promoting discussion within service

To provide complex information to the patients, clinicians and other agencies to assist in decision-making, using persuasive, motivational, negotiating, training, empathic and re-assurance skills.

To work with a defined caseload of patients with a broad range of health needs, using knowledge to carry out assessments, using standardised tools, setting objectives and implementing appropriate treatment, as part of the holistic care plan.

To formulate an individual management/treatment plan for each patient using clinical reasoning skills and utilising a wide range of physical, sensory and cognitive treatment skills following analysis of information received from client, carers and referrer and any other professionals involved in the patients care as necessary.

• To ensure own and other staffs interventions are evaluated and treatment is modified in order to measure progress and ensure effectiveness of intervention
• To provide spontaneous and planned advice, teaching and instruction to relatives, carers and other professionals to promote understanding of aims of therapy to ensure a consistent approach to patient care
• To have working knowledge of equipment, minor adaptations and resources including activities of daily living equipment, therapy and rehabilitation aids, moving and handling equipment and a basic knowledge of wheelchair provision in order to compensate for unresolved functional deficits, in line with local resources, guidelines and legislation.
• Proactive participation in multidisciplinary team meetings
• To complete clinical documentation and electronic data entry to a high professional standard and in accordance with Trust policies
• Promote confidence in the patient by means of appropriate professional standards of appearance, behaviour and attitudes
• Act on unplanned situations using appropriate clinical judgement
• Safeguard the confidentiality of all information regarding the patient and their family

Leadership

• To support the staff through the provision of clinical supervision, audit, appraisal, provision of support training and to be visible to staff, patients and relatives.

To act as an effective role model to other staff, being clear about expectations and ensuring that safe patient care is central to all activities

To act as mentor to newly qualified, new staff and other learners

• Provide clinical supervision/support to students and more junior members of staff
• Actively participate in audit and research
• Maintain and develop own clinical and leadership skills to support practice.
• Adhere to Health Care Professionals Council (HCPC) and College of Occupational Therapy (COT) Professional Standards and Conduct, Organisational Policies and guidance.
• Demonstrate a sound understanding of Clinical Governance and Risk Management and apply to work situation.
• To develop and support evidence to meet the Care Quality Commission (CQC) fundamental standards

Clinical Governance

Contribute to clinical governance reports covering all aspects of activity. Actively participates in audits, monitoring of care and reporting of activity as requested, and as a result of the team identifying areas of concern, or areas for development. Ensure that evidence is available and updated to demonstrate the Care Quality Commission fundamental standards are being met.

Professional/Personal

Adhere to the HCPC code and use this as a management tool to assist with monitoring the conduct of juniors. Maintain own professional integrity, being respectful and honest at all times. Ability to adopt the

management/leadership style required for any given situation. Maintain own knowledge and skills in order to be an effective clinician. Work positively with the Multidisciplinary Team ensuring that the team focus on the needs of the patient.

Professional Development and Education

Ensure that the clinical environment and team promote learning and development of each other and visiting students of all disciplines. Encourage and facilitate team members to develop professionally and meet the competencies set for their role. Participate in undertaking minstaff appraisals.

Communication/ Interpersonal Relationships

Competent with Information Technology (IT), systm one or generic case management software and able to manage own emails and online training. To assist with developing highly organised systems to ensure reporting/reviewing of data and audit results, ensuring action plans are written and achieved where needed. Actively participate in projects across the Organisation.

Other Responsibilities

The post holder will be expected to participate in a rota system of shift working to cover a 6- day a week service.

To travel between ECCH sites for meetings and training.

Qualifications

Essential

• Degree or equivalent in Occupational Therapy
• Registration with HCPC
• Evidence of continued professional development and a professional portfolio

Desirable

• Post registration qualifications in Occupational therapy interventions
• Evidence of management and leadership development

Experience

Essential

• Evidence of clinical experience relevant to the post

Desirable

• Previous experience of working as a band 6
• Experience of working within the community
