Job summary
An exciting opportunity has arisen for an Occupational Therapist to join our Primary Care Home team delivering high quality Therapy at home to patients registered with GP surgeries in South Waveney. The PCH team works closely with GPs and other members of the multidisciplinary team to provide an excellent standard of person centered care to adult patients in their own homes, residential care or clinic settings, supporting patients to self-manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working.
The successful candidate will be accountable for the effective management of a caseload, ensuring the delivery of high standards of evidence-based therapy input. They will demonstrate excellent leadership, organisational and communication skills and the ability to influence and manage change.
Occupational Therapy is challenging, varied and rewarding; the service has to be flexible to meet the needs of patients. Main duties of the job
The Post offered is part time and sits in a multidisciplinary team providing community services 7 days per week 07:00-20:30 hours.
The role contains variety every day with a mixture of face-to-face clinical practice, telephone assessments, clinical triage, leadership and management responsibilities.
The main duties of the role are: -
- To lead on the reablement of patients to ensure the patients needs and goals are met, regularly liaising with team members to progress rehab programs relevant to changes in the patients condition, this will range from reacting to patient in crisis, palliative patients, to supporting discharge and admission avoidance.
- You will have a focus on optimising independence in activities of daily living.
- To work autonomously at a level appropriate to clinical competence and within scope of professional practice.
- To carry out rapid assessments in patients own home with the aim of preventing admission to the acute hospital. These assessments will take a holistic ethos including physical, physiological and social aspects of need.
- To be an autonomous practitioner that is responsible for own case load and to take a lead in the delivery of projects to improve service delivery, patient safety and quality of care.
- To contribute to the continuous evaluation of the service to ensure it is fit for purpose.

and students, with assistance from more senior members of the team. This will include the use of formal appraisal
documentation.
- To provide
teaching/training to other members of the Multi-Disciplinary Team.

in the collection of appropriate data and statistics for the use of the
service.
- To
deputise for Band 7 in terms of operational issues when appropriate.
