An exciting new opportunity for an experienced Physiotherapist has arisen to work as part of a therapy team at Carlton Court intermediate inpatient care unit based in Lowestoft.

The post holder will be expected to provide specialist clinical skills to manage a caseload of patients using evidence based practice in order to achieve their goals.
Main duties of the job
The post holder will provide a high standard of care as part of a multi-disciplinary team to patients within the intermediate care service and patients own homes. The post holder will be working as an autonomous practitioner applying a high level of clinical reasoning to ensure that individualised care is delivered safely.
The post holder will be responsible for delivering an Intermediate Care service for patients with highly complex needs within a ward-based setting however there may be a requirement to work across a wider community setting. The emphasis of the service will be reablement of patients in order to promote:
- Faster recovery from illness
- Prevention of unnecessary acute hospital admissions
- Prevention of premature admission to long term residential care
- Support for a timely discharge from hospital
- Maximise independent living
Develop and maintain clinical skills in line with the roles job description and specific professional registration.
Provide individualised care plans for a complex patient caseload and ensuring that any delegated workload is completed by staff with the appropriate competencies.

Accountable
for care standards and the delivery of care promoting a culture of continuous
improvement of clinical standards with particular emphasis on patient
experience and key clinical outcomes.
Involve patients in the planning of care wherever possible, and act in
the best interests of the patients at all times. Actively manage the patients
admission, involving clinical experts, when relevant, to advise in the
diagnosis and treatment. Proactive discharge management, ensuring patients are
consulted and proper management of situations where patients need assistance in
decision making.

interpret and analyse clinical and non-clinical facts to form an accurate
diagnosis and prognosis in a wide range of conditions, to recommend the best
course of intervention and to develop comprehensive management strategies for
the patients presenting condition.

enhance and develop the skills and dexterity, co-ordination and palpatory
senses for assessment and manual treatment of patients. To be safe and
competent in the use of therapy equipment.

refine treatment techniques in line with the most recent evidence of clinical
effectiveness by evaluating and implementing key developments and promoting
discussion within service.

provide complex information to the patients, clinicians and other agencies to
assist in decision-making, using persuasive, motivational, negotiating,
training, empathic and re-assurance skills.

work with a defined caseload of patients with a broad range of health needs,
using knowledge to carry out assessments, using standardised tools, setting
objectives and implementing appropriate treatment, as part of the holistic care
plan.

formulate an individual management/treatment plan for each patient using
clinical reasoning skills and utilising a wide range of physical, sensory and
cognitive treatment skills following analysis of information received from
client, carers and referrer and any other professionals involved in the
patients care as necessary.

modified in order to measure progress and ensure effectiveness of intervention.

provide spontaneous and planned advice, teaching and instruction to relatives,
carers and other professionals to promote understanding of aims of therapy to

have working knowledge of equipment, minor adaptations and resources including
activities of daily living equipment, therapy and rehabilitation aids, moving
and handling equipment and a basic knowledge of wheelchair provision in order
to compensate for unresolved functional deficits, in line with local resources,
guidelines and legislation.
