Care Coordinator - Primary Care Home
An exciting opportunity has arisen for a Care Coordinator to join our Primary Care Home (PCH) to deliver high quality care to the population of Gorleston and wider Great Yarmouth and Waveney area.
To support the senior triaging clinician with delegated responsibilities actively coordinating the multidisciplinary management of patients within the local community and ensuring referrals and visits are managed in a timely manner, including urgent community responses.
To have an understanding of the wider health and social care systems to enable a high standard of communication and information to referrers into the service.
This role will also support the PCH teams with administrative tasks such as supporting the lone worker process.
The PCH is a multidisciplinary team working closely with GP's and other community partners to provide an excellent standard of person centered care to adults in their own homes, residential care or clinic settings, supporting patients to self-manage whenever possible. The service uses SystmOne as a clinical IT system to support mobile working.
The post is offered at 30 hours per week and sits in a multidisciplinary team providing community services 7 days per week 07:00- 20:00 hours.
Main duties of the job
As a Care Coordinator you will be based at Yare house within Gorleston PCH. You will work in partnership with other members of the multidisciplinary team to coordinate and triage planned and reactive care within the community services.
The post holder is required to work autonomously at a level appropriate to clinical competence and within scope of professional practice.
The post holder will work under the supervision and in partnership with registered staff providing and support delivering high standards of care contributing to the on-going assessment, provision and monitoring of care based on evidence and working in partnership with the multidisciplinary team.
