The Post Covid Community Pathway and Pulmonary Rehabilitation (PR) Services in Bromley are looking for an experienced and enthusiastic Specialist Physiotherapist who would like to be involved in developing these services.

The Post Covid Community Pathway is going through an exciting time of developing into a multiple symptom pathway including patients with fatigue, breathlessness or "brain fog" and other potentially related symptoms such as pain, dizziness or cough. If you are a Physiotherapist, with respiratory experience who would like to use their skills to work alongside the multi-disciplinary team, but also assist with service development and quality improvement, please get in touch.

The PR service is currently working towards national accreditation. Under the NHS 5 year plan, respiratory services were identified as a key target for the NHS to aim to reduce hospital admissions. Pulmonary Rehabilitation was identified as a key component to promoting the health and wellbeing to those patients with a chronic lung condition.

This role will predominantly be based in these two services. However, there will also be an expectation for the post holder to support other respiratory services in the organisation as required.

Main duties of the job

The Specialist Physiotherapist is responsible for the day-to-day professional management of all aspects of Physiotherapy intervention to people referred to the Post Covid Syndrome Pathway and Pulmonary Rehabiliation services within Bromley Healthcare delivered to patients either in their own homes, at clinics and classes, or by virtual means.

The post holder is expected to be a positive role model and an effective leader in terms of clinical management, patient care, communication and quality standards. The post holder is expected to be a clinical specialist in their respective field and to provide clinical management, advice, support and teaching to all relevant personnel within the organisation.

The post holder is expected to provide a comprehensive Physiotherapy service to the patients referred with complex multiple pathologies who exhibit a range of physical, psychological, neurological, emotional, and social problems. To provide a Physiotherapy service of the highest possible standard for those referred to the services by:

Clinical:

  • Triaging referrals to the Post Covid Syndrome Pathway, discussing these with the MDT to ensure all clients receive intervention from the most appropriate services.
  • To undertake full physio assessments and independently plan individual, specialised, evidence-based, treatment programmes for patients appropriately using clinical reasoning and a variety of highly specialised skills such as use of respiratory monitoring, exercise prescription and exercise progression.
  • To demonatrate expertise in the use of respiratory physiotherapy skills to include auscultation, airway clearance techniques and assess exercise tolerance.
  • To assess breathing pattern disorders and provide intervention as clinically indicated.
  • Agreeing goal orientated programmes in conjunction with the client, his/her family and other professionals involved and planning and implementing appropriate intervention.
  • Devising rehabilitation programmes using the most up to date available evidence and tailoring for common co-morbidities in Post Acute Infection Syndrome.
  • Liaising with other health care professionals, making referrals and recommendations where necessary to reduce the effect of disability on health and wellbeing.
  • Participate in health promotion/education and give advice as necessary to clients/their carers and provide guidance to enable clients to develop skills that will improve their performance, independence and wellbeing.
  • To formulate and evaluate individualised management and / or treatment plans, using clinical reasoning and utilising a wide range of treatment skills and options in order to formulate a specialised programme of care e.g. supported Self-management.
  • Monitor clients health and medical status; reporting any changes or deterioration to the MDT involved in the clients care accordingly.
  • To attend and on occasions lead the weekly MDT meetings to discuss clients.
  • To carry a caseload of patients as an autonomous practitioner, including those with highly complex presentations, using advanced clinical reasoning skills.
  • To provide support, supervision and delegate clinical work to Junior staff and the Therapy Assistant in order to meet needs of the client group.
