Childrens Nurse - Band 6
£44,485 - £52,521 Pro Rata Per Annum inclusive of HCAS
To be responsiblefor the caseload within the Children's Community Nursing Team, providing specialist community nursing care, advice, and support to children and their families within the home, school, or other locations.
Act as a resource within this specialist area across Health, Social Care, and Educational agencies, thereby promoting holistic family care.
Be responsible for the day-to-day management and delivery of care being carried out by the other members of the nursing team.
Be a key health professional responsible for meeting the health needs of identified pupils with complex chronic health conditions within schools, and other locations, planning and leading the implementation of individual care plans for the schools and other locations, liaising with fellow professionals, education staff, parents/carers, and other agencies as necessary.
Additionally, the children's community nursing team will offer a comprehensive palliative care package to children as needed, so the post holder will also be expected to participate in this 24-hour on-call service - for end-of-life care only.
This post involves participation in the rota for a Monday-Friday 8 am-6 pm service and a pattern of shift work, which may include covering 9-2 pm weekends and bank holidays if clinically needed.
Main duties of the job
To adhere to the Nursing and Midwifery Council Code of Professional Conduct and to be conversant with the scope of Professional Conduct and other Nursing and Midwifery Council Advisory papers
The post holder is expected to work within and practice according to all Bromley Healthcare's relevant policies, procedures, and guidelines, including the appropriate Code(s) of Conduct associated with this post.
To take responsibility for one's own professional and organisational knowledge in accordance with relevant statutory and local requirements, including revalidation.
To demonstrate active interest and lead the team in at least one key area of community children's nursing.
To work closely with staff from statutory organisations as well as parents and carers so that these adults can meet the daily needs of the patients/pupils to enable children to have access to education and other activities of daily living.
To ensure that the relevant Safeguarding Policies and Procedures are followed and that these are available to and understood by self, junior staff, and colleagues.
To participate in Bromley Healthcare's Clinical Supervision programme both for self and for other staff.
To act as a resource person for statutory and voluntary agencies with specific regard to the roles and responsibilities of the CCNT

To maintain close contact with hospital teams and other Primary Health teams and providers, which could include conferring regularly with consultants, general practitioners, attending, and participating in internal and external meetings.

To assist in the development of joint protocols and care pathways in conjunction with both hospital and Multi-Disciplinary Team professionals.

To assess, plan, implement, and evaluate programmes of care that will enable children to be nursed at home and other locations closer to home as an alternative to hospitalisation, and to support and enable parents as partners in care.

To facilitate a multi-disciplinary approach to the management of children within the caseload.
To provide specialist nursing input when required, ensuring all medical and nursing needs are understood and implemented in accordance with relevant standards and protocols.
To be responsible for a special interest role in a long-term condition. If required, training will be provided.
To establish and maintain good working relationships with the child and parent/carer and others involved in the child patients life at all times, including statutory and voluntary organisations.
To play a key role in liaising with all hospital departments to promote the home care philosophy for children with health-related needs. To provide training for school staff and other care staff to enable them to safely undertake and meet the care needs of the children, as directed. This could include training them to undertake feeding via a gastrostomy tube, oro-pharyngeal suctioning, administration of daily medications and emergency medication, and any other care needs as required. This includes reassessment of individual competence.
Participation in the monitoring and assessment of the childrens continence needs.
Administer prescribed medication and treatments according to Bromley Healthcare policies and procedures. Record all treatments and medications given.
To participate in the national schools immunisation programme within the special school setting.
To accept referrals from both hospital and community services according to local procedure.
To make referrals as appropriate to other members of the health care team and other agencies, ensuring that ongoing support is offered and available.
