Job summary

One Bromley - High-Intensity User Lead - Band 7

£49,178 - £55,492 Inc HCAS per annum pro rata

Full-time - 37.5 hours per week

A relatively small percentage of individuals are known to generate a disproportionately high percentage of Emergency Department attendance, hospital admission and contact with primary care, along with a higher number of calls to 999, ambulance and 111.

Individuals in this high intensity user (HIU) group may have unmet social needs that present as medical or mental health episodes or be experiencing homelessness, substance abuse or self-harming and are more likely to experience poor health outcomes.

The HIU Lead will work directly with the HIU client group, delivering a service with a highly personalised approach that supports improved wellbeing and health outcomes, whilst reducing inappropriate contact with health care services and unscheduled care in particular. The HIU Lead will lead a service with a focus on supporting the HIU client group to flourish through sustaining job opportunities, reconnecting with families, improving well-being etc.

Main duties of the job

The HIU Lead will lead on the identification and reduction of high intensity use (HIU) across the local system and the transformation of culture, behaviours and attitudes of high intensity users of health care. The postholder will also lead and coordinate efforts to integrate HIU working practices amongst partners and key stakeholders, to measurably improve system working and outcomes for the benefit of service users, staff and the public.

The post holder will line manage and undertake work planning with an HIU coordinator who sits at Band 6 within Kings College Hospital Trust.

One Bromley is the Local Care Partnership for Bromley, co-ordinated by a small team hosted by Bromley Healthcare. The partnership is led by a number of key senior officers of the Bromley system and includes all the main NHS providers in Bromley, London Borough of Bromley, Bromley

Third Sector Enterprise, St Christopher's Hospice and SEL CCG (through Bromley borough based structures).

This group is the One Bromley Executive - the senior management team providing oversight and support for the local health and care system, initially focused on Urgent & Emergency Care, and the further development of community services to reduce acute service use and promote good flow through the system, as well as specific areas such the Proactive Care pathway and Frailty pathways.

One Bromley is part of SEL ICB, and works as part of an overall Integrated Care System in South East London
