Complex Case Lead - One Bromley - band 8A

£56,388 - £62,785 Inclusive of Hcas

This role will be the clinical lead for hospital discharged clients for continuing healthcare, (CHC) discharge to assess (D2A) and fast track pathways and for patients who are likely to be eligible for joint funding due to their health and social care needs.

Health & Social Care organisations in Bromley have agreed to work together to enhance and improve the range, quality and effectiveness of services available to local people, with the aim of enabling partners and services in Bromley to work as a single system to deliver integrated care.This approach is based on the premise that patients need support from health and care professionals that act as one team and work for organisations that behave as one system

About us

One Bromley is the Local Care Partnership for Bromley, co-ordinated by a small team hosted by Bromley Healthcare. The partnership is led by a number of key senior officers of the Bromley system and includes all the main NHS providers in Bromley, London Borough of Bromley, Bromley Third Sector Enterprise, St Christopher's Hospice and SEL CCG (through Bromley borough based structures). This group is the One Bromley Executive - the senior management team providing oversight and support for the local health and care system, initially focused on Urgent & Emergency Care, and the further development of community services to reduce acute service use and promote good flow through the system, as well as specific areas such the Proactive Care pathway and Frailty pathways.OneBromley is part of SEL ICB, and works as part of an overall Integrated Care System in South East London

Qualifications

Essential

  • Advanced Nurse Practitioner/community matron
  • Post graduate qualification relating to service
  • Mentorship SSSA or equivalent
  • Management/leadership qualification

Professional Registration

Essential

  • Current NMC registration - RN/RGN

Specific Skills

Essential

  • Present highly complex, sensitive and contentious information
  • Experience of dealing with safeguarding cases
  • Expertise in proactive and robust performance management of colleagues including both capability and conduct issues
  • Competence in addressing safeguarding issues including a detailed understanding of mental capacity
  • Advanced negotiation and problem solving skills with clients/ carers, staff and other professionals, including effective handling of complaints
  • Experience of being a budget holder and manage resources effectively (Financial and human)

Experience

Essential

  • Highly experienced ANP/CM underpinned by a sound theoretical framework
  • Experience of successful change management
  • Significant experience at a senior clinical and leadership level
  • Experience of supervising/ appraising staff, students and working with the clinical governance framework
  • Appropriate experience to lead in clinical areas.
