Job summary
37.5 hours per week.
This is an exciting opportunity for an experienced band 6 nurse to join the Care Home Service, an integrated team within Livewell Southwest on a 12 month secondment / fixed term contract. This role requires specialist nursing expertise to undertake and be responsible for completion of timely and comprehensive holistic evidence-based assessments, in a range of settings, and to apply the relevant nationally approved criteria to establish eligibility for services and care.
The successful applicant will need be response to system pressures & complete a range of tasks, working flexibly & interchanging between hospital and community settings.

Main duties of the job
Undertake timely, complex & detailed assessments. Prepare reports, plans of care, complex papers or tools such as the DoH Checklist & Decision Support Tools.
Work in a collaborative manner with HSC colleagues in the co-ordination & best interest of the client.
Work in collaboration with the private sector Care Homes to facilitate staff accessing appropriate education opportunities.
Provide specialist assessment skills, knowledge & management of patients in the hospital/community whilst ensuring the delivery of high calibre care, & efficient use of facilities, manpower & other resources.
Be able to confidently liaise with all members of MDT & participate in patient case conferences as required.
Liaise with community teams, & other professionals to promote person centred care planning appropriate to the complex needs of individuals, ensuring arrangements are made for input/training/follow-up with specialist teams where necessary.
Work with teams within LSW, UHP and Devon Integrated Care Board, in developing processes. Work within agreed assessment formats with adherence to established pathways.
Work with PCC Adult Regulated Care Team, Care Home Service & other key stakeholders to facilitate the further development & implementation of best practice & communications within the Care Home community.
Work in partnership with care home & other key stakeholders to escalate & address any concerns raised.
