An
exciting opportunity has arisen for a Adult Social Worker to join our newly
formed frailty team at Livewell Southwest.
The
successful applicant will work collaboratively with Livewells community
integrated teams and the Primary Care Networks across the city as a core member
of the Ageing Well Multidisciplinary Team (MDT), with the ability to facilitate
optimal outcomes and reduce avoidable hospital admissions through promoting
independence and improving quality of life.
There will be opportunities for upskilling and skill sharing for
the successful applicant, although we also recognise the desire for individuals
to maintain their own professional registration and maintain their
competencies.
Main duties of the job
- Key member of the Ageing Well Multidisciplinary Team (MDT), Supporting the Frailty practitioner and Livewell community integrated teams in completion of comprehensive geriatric assessment, working in conjunction with primary care network colleagues.
- To utilise specialist adult social worker skills and knowledge to support with completing comprehensive geriatric assessments as well as taking the lead on assessments to support service users safety and independence in their own home.
- To be able to consider adult social care input as a part of a wider holistic approach to treating patients with frailty and recognising where boundaries can be blurred between professionals in order to facilitate optimal care.
- To participate in the planning, development and evaluation of the ageing well MDT. This will include weekly attendance at multiple MDTs with the ability to provide a voice for adult social care within the MDT discussion.
- Wherever possible the post holder is required to support people to take risks in complex situations so that their outcomes and wishes are promoted.
