Job summary
We are seeking an experienced Band 6 Health Screening & Community Nurse to join our Community Learning Disabilities Team on a fixed-term contract until February 2027 for maternity cover. Full-time (37.5 hrs) with a blended focus
Three days per week delivering health screening services, focusing on supporting service users, families & care providers to engage with the five national screening programs across settings. Your role will include advocating for reasonable adjustments, using enhanced communication skills & creating pathways to ensure equitable access to screening. You will also deliver specialist educational support & resources to mainstream services, promoting inclusion & better health outcomes
Two days per week providing community nursing care as part of a multidisciplinary team (MDT). This senior clinical role is accountable for clinical effectiveness & service development for individuals with complex needs. You will deliver holistic care, covering both physical & mental health, through case management, person-centred planning & coordination of support. You will liaise with agencies and providers to ensure adjustments & continuity of care

Main duties of the job
Health Screening Role (3 days per week) This post focuses on improving uptake of the five national screening programmes for individuals with learning disabilities and supporting timely access to checks that promote early identification and treatment. You will work with GP surgeries, hospitals, screening teams, and third-sector organisations to reduce health inequalities. Working alongside National Screening Leads, Learning Disability Services, Care Coordinators, and clinical leads, you will review GP LD registers, identify cohorts for proactive contact, and use innovative approaches to raise awareness of screening programmes and their role in improving health outcomes.
Community Nursing Role (2 days per week) As part of the MDT, you will assess, plan, and manage risks for individuals care needs, including physical and mental health and behaviours that challenge services. This senior clinical role includes case management and care coordination for those fully health-funded, liaising with commissioners to ensure needs are met. You will advocate for individuals, ensuring reasonable adjustments by mainstream services, and work in partnership with families and providers while maintaining safeguarding and best-interest decision-making in line with legal frameworks.
