Join a committed and compassionate team delivering out of hours nursing care to patients in their own homes across Plymouth, South Hams, and West Devon. This part time role (up to 36 hours per week) is ideal for an experienced and motivated nurse who thrives in autonomous practice and is eager to contribute to the growing Urgent Care Nursing Service.
You will work a combination of twilight shifts (17:00 23:00 or 18:00 00:00) and night shifts (12 hours with a 1 hour unpaid break), providing high quality, patient centred care that supports individuals to remain safe, independent, and well at home.

What You will Do
- Deliver high quality, evidence based nursing care to acutely unwell patients in their homes.
- Provide end of life care and symptom management to patients in their homes.
Assess, plan, implement, monitor, and evaluate care to meet individual patient needs.
This role may not be eligible for sponsorship under the Skilled Worker route.

All staff are expected to be able and willing to work across a 7 day service.
Main duties of the job
- Duties:
- Deliver high quality, evidence based nursing care to acutely unwell patients in their homes.
- Provide end of life care and symptom management to patients in their homes.
- Assess, plan, implement, monitor, and evaluate care to meet individual patient needs.
- Liaise with referring clinicians when accepting patients for treatment, with support from senior clinicians and the clinical manager.
- Provide advanced and intensive nursing interventions to help prevent hospital admissions and reduce hospital length of stay.
- Work collaboratively with the wider multidisciplinary team to ensure safe, effective care.
- Administer intravenous therapy (full training provided). When required.
- Subcutaneous and intramuscular injections, including diabetic therapies.
- Administration of prescribed medications
- Acute and chronic disease management.
- Acute and chronic pain management.
- Palliative and endoflife care in collaboration with the multidisciplinary team.
- Continence management and bowel care.
- Facilitating appropriate elimination care, including catheter and bowel management.
- Undertaking and interpreting acute observations using tools such as NEWS2 and ABCDE.
- Blood glucose monitoring.
- Pulse oximetry and interpretation.
- Blood tests for monitoring and review.
- Undertaking ECGs and other investigative procedures to support diagnosis.
