Job summary

37.5 hours per week. The postholder will work as part of a multidisciplinary team taking referrals, participating in biopsychosocial assessments and providing treatment interventions for patients 18 and over who are experiencing mental health crisis.

We are now looking to recruit an exceptional Community Support Worker to join our growing team. We are very interested in hearing from you if you have a commitment to supporting people to remain at home during their illness along with a positive can-do attitude.

We will particularly welcome applications from people with acute mental health experience, older adult experience (including dementia) or from other health sectors where transferable skills can be demonstrated.

This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.

Main duties of the job

Visits to the inpatients units and exposure work is an important part of the role. Delivery of medication, physical checks and group work are all part of routine operation within the team. Good risk assessment skills are vital. Some autonomous working may be required at times and high quality communication skills are essential. Home Treatment has been established in Plymouth for over 20 years and is a 24/7 service. The purpose is both to prevent unnecessary admissions and facilitate early or prompt discharges from Livewell's inpatient functional wards.

2.Knowledge, Training and Experience

a)The post holder will participate in updating their practice; knowledge and skills within their sphere of practice, under the guidance of qualified staff, including LWS mandatory training and Mental Health essential training e.g. Mental Health Act and Care Programme Approach.

b)Vocational Level 3/NVQ 3 e.g. Certificate in Mental Health at level 3 or equivalent and commitment to further education.

c)Experience of working with people with mental health problems.

d)Experience dealing with a variety of mental health problems in various settings both in the community and in hospital with support and supervision from qualified staff.

e)An awareness of Policy and legislation and how to access the information both in writing and electronically.

a)Liaise with other services to arrange appointments that both meet patient and service demands.

Identifying car, phone and bleep.

Complete White Board (lone worker policy)

Collect medication to be delivered from qualified nurse.

Implement a variety of packages of care as outlined in the individual patients care plan.

b)Competent keyboard skills to enable information to be inputted into System one and eCPA. Must be able to keep electronic diary up to date and confidence to take on more IT skills as clinical technology improves.

c)Ability to take accurate physical observations i.e. blood pressure, pulse and temperature and record in notes advising qualified staff if there were any problems. E.g. starting someone on Clozapine will require close monitoring of physical condition in line with CPMS guidelines.

d)Home visits to multi-occupied buildings, many without lifts.

e)Escorting patients out of their home environment providing programmes of care and ongoing assessment of need. (e.g. Anxiety Management Programmes).

f)Driving frequently to home visits throughout the city.
