Are you passionate about making a real difference in peoples daily lives?Join our dedicated Continence Service as a Healthcare Assistant and play a vital role in supporting individuals across Plymouth, South Hams and West Devon to manage their bladder and bowel health with dignity, confidence and compassion.
In this rewarding community-based role, youll bring your specialist continence knowledge and developing competencies to patients and carers in their own homes, residential and nursing settings, and local clinics. Youll be part of a supportive multidisciplinary team responding to referrals from DDRS, Primary Care and a wide range of professionalsmaking every day varied and meaningful.
This role offers exciting opportunities to teach and empower both colleagues and patients, through formal education sessions and informal coaching. You will promote self-care, encourage independence, and support the use of digital technology to enhance continence management.
Working Monday to Friday, 08:30-17:00, you'll help improve quality of life for people experiencing bladder or bowel incontinence providing sensitive, expert support that truly transforms daily living.

Main duties of the job
You will work autonomously within your defined competencies, carrying out delegated clinical tasks in line with local policies & procedures
Working across a defined geographical area, you will support patients in their own homes, care homes, clinics & other community settings. Responsibilities include assessing care needs, monitoring for changes in clinical condition, & reporting findings promptly to the appropriate registered professional
Delivering effective, patient focused HCA support, prioritising tasks & following treatment plans as directed by registered staff. A holistic approach to patient care is essential, alongside maintaining accurate & timely clinical records. Participate in all mandatory & role specific training, including safeguarding & responding to urgent situations, & complete clinical competencies relevant to your service area Flexibility is required to work within LSW policies, undertake clinical & clerical duties relevant to the service, & support wider organisational needs.
Ability to frequently travel independently and in a timely manner between Livewell Southwest sites and community locations including areas not served by public transport.
Contributing to planning, monitoring & evaluating patient centred care, escalating concerns appropriately. Following assessment by a Registered Nurse or AP, you will implement planned programmes of care and act as a positive role model for colleagues and students.
