Join a dedicated team providing intravenous therapies in patients' homes across Plymouth, South Hams, and West Devon. This full-time role (up to 32 hours per week, is ideal for an experienced and motivated nurse eager to contribute to a growing Urgent Care Nursing service.
You will deliver high-quality, patient-centered care in the community, helping individuals remain safe and independent at home while supporting their health and wellbeing. The role involves utilising clinical skills and expertise within the Intravenous Therapy Day Service.
We seek a nurse with strong clinical knowledge, sound judgement, and the ability to work autonomously and collaboratively in a forward-thinking team.

Main duties of the job
You are:
: responsible for the delivery of high quality evidence based care for acutely ill patients within their homes.
:will deliver intravenous therapies for patients within our service, training will be given.
Responsible for effectively assessing, planning, implementing, monitoring & evaluating patient care.
To prevent & reduce hospital admissions & minimise hospital lengths of stay by providing intensive & advanced nursing input, & involving appropriate members of the multi-disciplinary team.
Responsible for coordinating a shift in the absence of a senior clinician
The Urgent Care Nursing team work on a flexible rota shift system. The service covers 24 hours per day over seven days a week. However the Intravenous therapy service runs from 08:00 - 20:30, this post is primarily shorter shifts either 7.5 hour or 9.5 hours dependant of needs of business shift per day which runs from 08:00 each day.
The post holder will need to travel and attend to service users based across Plymouth and other geographical areas. The ability to frequently travel independently and in a timely manner, between Livewell Southwest sites and community locations, including areas not served by public transport, is essential.
Able to provide the interface between hospital, primary care, community & social care settings
