We are offering an exciting opportunity to join our innovative Community Frailty Virtual Ward, delivering high-quality, patient-centred care in patients own homes. Our virtual ward supports people to remain safely at home, reducing hospital admissions, shortening length of stay and improving wellbeing through enhanced multidisciplinary working.
As an Advanced Clinical Practitioner, you will use your expert skills in assessment, diagnostics and clinical decision-making to provide autonomous care for patients at risk of deterioration. You will work closely with geriatricians, GPs, Nurses, AHPs and partners to ensure proactive, coordinated support. You will also contribute to service development, leadership, education and continuous improvement within this evolving model of care.
We welcome applications from experienced clinicians with an MSc in Advanced Clinical Practice (or equivalent) who are motivated, compassionate and confident working in a dynamic community environment.
All Livewell Southwest staff are expected to work flexibly across a 7day service.

Main Duties of the Role
As an Advanced Clinical Practitioner, you will be a key member of our new multidisciplinary Community Frailty Virtual Ward, working across one or more of the six virtual wards across Plymouth South Hams and West Devon. You will provide specialist assessment, intervention and coordinated care for people living with frailty, working closely with Consultants, GPs, ACPs, Pharmacists, Frailty Practitioners, Clinical Nurse Specialists, Occupational Therapists, Physiotherapists, Frailty Nurses, Assistant Practitioners, Nursing Associates and HCAs to deliver safe, integrated support for all patients admitted to the service.

Key Responsibilities
- Deliver advanced, autonomous clinical assessments and interventions for patients on the Frailty Virtual Ward.
- Provide leadership within the virtual ward and support inreach to the acute hospital when required.
- Manage complete episodes of care alongside medical colleagues.
- Undertake comprehensive history taking, clinical examination and frailtyfocused assessment.
- Diagnose, plan and implement evidencebased treatment.
- Prescribe safely in accordance with professional frameworks.
- Request, interpret and act upon diagnostic investigations.
- Make appropriate onward referrals and ensure effective planning, review and evaluation of care.

Working
autonomously as an Advanced Clinical Practitioner within the clinical team and

Lead

The
post holder will be working across organisations and communicating with a range
